摘  要:目的探讨超敏C反应蛋白(hs-CRP)、肌钙蛋白I(cTnI)在新生儿缺血缺氧性脑病(HIE)中的变化及其意义。方法利用全自动生化分析仪测定30例HIE和18例正常新生儿血清hs-CRP、cTnI水平,并同时检测血清CK、CK-MB、LDH、ALT、AST。结果HIE新生儿急性期血清hs-CRP水平明显高于对照组(P<0.05);恢复期下降,与正常对照组差异无显著性(P>0.05)。HIE新生儿急性期血清cTnI水平明显高于对照组(P<0.01);恢复期cTnI与正常对照组之间无明显差异(P>0.05)。HIE新生儿急性期CK、CK-MB、LDH、AST、ALT均较正常新生儿升高;恢复期CK、CK-MB、ALT降低明显(P<0.05),与正常对照组之间差异显著(P<0.05);LDH、AST降低不明显(P>0.05),但与正常对照组比较差异仍显著(P<0.05)。结论hs-CRP和cTnI的动态变化与HIE的疗程相平行,能较敏感地反映HIE患儿病情程度及转归趋势,与血清酶学改变一起可作为了解HIE的疗效及病情监测的指标之一。Objective To investigate the changes of high- sensitivity (3 reactive protein (hs -CRP) and cardiac troponin Ⅰ (cTnⅠ) levels in newborns with hypoxic- ischemic encephalopathy (HIE). Methods Thirty infants with HIE and 18 healthy newborns were selected as study cases and controls respectively by automatic biochemistry analyzer. Serum hs- CRP and cTnl contents were measured by reaction rate method: and the activities of creatine kinase (CK), creatine phosphokinase iscenzyine (CK - MB), lactate dehydrogenase ( LDH), alanine transaminase (ALT), and aspartate aminctransferasc (AST) were detected as well. Results The hs- CRP level in newborns with HIE at acute stage was significantly higher that that of the controls ( P〈 0.05). The hs- CRP level in newborns with HIE at convalescent stage decreased, and there was no significant difference between the newborns with HIE and the controls (P〉0.05). The cTnl level in newborns with HIE al acute stage was significantly higher than that of the controls ( P〈 0.01), and there was no significant difference on the cTnl level at convalescent stage between the newborns with HIE and the controls (P〉0.05). The values of CK, CK - MB. LDH, ALT. and AST in newborns with HIE at acute stage were higher than those in the controls: the values of CK, CK - MB, and ALT decreased significantly at convalescent stage (P〈 0.05). there were significant differences between the newborns with HIE and the controls (P〈 0.05). But the values of LDH and AST in newborns with H E decreased insignificantly at convalescent stage ( P〉0.05), and there were still significant differences as compared with the controls (P〈0.05). Conclusions The variation of hs- CRP and cTnl levels in newborns with HIE is paralleled to the course and severity of the disease, which can sensitively reflect the fate and prognosis of HIE. Therefore, hs- CRP and cTnl may be used as the vital indexes for evaluating curative effect and judging prognosis
