摘  要:目的:许多研究报道青蒿素能抑制多种肿瘤细胞的增殖活性,临床证实青蒿素对增殖性瘢痕也有抑制作用,但是其机制不详。实验拟验证青蒿素对兔耳增生性瘢痕的影响。方法:实验于2006-09/2007-03在重庆市神经病学重点实验室完成。①实验分组:新西兰大白兔16只,体质量(2.5±0.3)kg,兔耳健全,雌雄不拘,随机数字表法分为对照组(n=4)和青蒿琥酯组(n=12);青蒿琥酯由广西桂林南药股份有限公司提供。②实验方法:建立兔耳增生性瘢痕动物模型,术后第6天,开始向创面基底部及创面灶内局部注射50g/L NaHCO3;青蒿琥酯组各选4只兔分别注射浓度为40,80,120g/L的青蒿琥酯钠。③实验评估:术后28,56d测定瘢痕发生率及瘢痕增生指数的变化;苏木精-伊红染色后光镜下观察瘢痕的变化情况。结果:纳入大白兔16只,均进入结果分析。①青蒿琥酯可以降低兔耳瘢痕发生率及瘢痕增生指数,与对照组相比,差异显著(P<0.05,P<0.01);其抑制瘢痕增生效果与青蒿琥酯浓度呈剂量依赖性。②术后28d光镜下见青蒿琥酯能明显抑制成纤维细胞增殖及降低胶原纤维的含量。③苏木精-伊红染色显示,青蒿琥酯组:真皮层变薄,成纤维细胞数量较少,胶原纤维密度下降,胶原纤维排列规则,无胶原结节或者胶原结节不明显。而对照组真皮层较厚,成纤维细胞密集,胶原纤维旋涡状,形成胶原结节。结论:青蒿琥酯可明显抑制兔耳创面增生性瘢痕的形成及瘢痕组织增生。AIM: Some studies have demonstrated that Artemisinin can inhibit the proliferation of many tumor cells, and Artemisinin has been proved to prevent and cure hypertrophic scar in clinical practice. But the mechanism is not detailed. This study is aimed to verify the inhibiting effect of Artemisinin on the hypertrophic scar in rabbit ears. METHODS: This study was carried out in the Key Laboratory for Neurology of Chongqing City between September 2006 and March 2007. Sixteen New Zealand rabbits of either gender, weighing (2.5±0.3) kg, were taken as experimental animals and randomized into experimental group (n=12) and control group (n=4). Artesunate were supplied by Guangxi Guilin Pharmaceutical Company. The hypertrophic scar models in rabbit ears were reproduced. The scars of every four rabbits in the experimental group received Artesunate by local injection in different doses of 40, 80, 120 g/L on postoperative 6th day. The scars of rabbits in the control group were treated with 50 g/L NaHCO3 by local injection into wound basilar part and wound site. On postoperative 28th and 56th days, the hypertrophic index and the rate of scars were detected. Hematoxylin-eosin staining was applied to observe the morphological change of scars under light microscope. RESULTS: Sixteen experimental rabbits were all involved in the final analysis. Compared with the control group, the hypertrophic index and the rate of scars in the Artesunate treated group were significantly reduced in a dosage-dependent manner (P 〈 0.05, 0.01). The proliferation of fibroblast was obviously inhibited and the contents of collagen fiber were decreased in the Artesunate treated group under light microscope on the postoperative 28th day. Hematoxylin-eosin staining results showed that, the dermises of the scars in the Artesunate treated group were thinner and contained few fibroblasts.
