摘 要:目的探讨老年2型糖尿病患者颈动脉内膜中层厚度(IMT)与血清高敏C反应蛋白(Hs-CRP)、血栓调节蛋白(sTM)以及其他动脉粥样硬化危险因素之间的关系。方法将89例2型糖尿病患者分为IMT增厚组(IMT>0.9mm)61例和对照组(IMT≤0.9)28例,分别检测血Hs-CRP、sTM、血脂、血糖等生化指标,并加以分析。结果IMT增厚组和对照组比较,收缩压、脉压、血尿酸(UA)、低密度脂蛋白胆固醇(LDL-C)、Hs-CRP差异有统计学意义,P<0.01,总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、sTM差异也有统计学意义,P<0.05,舒张压、空腹血糖(FPG)、空腹胰岛素(Fins)、血甘油二三酯(TG)、糖化血红蛋白(HbAlc)差异无统计学意义。单因素相关分析:IMT与收缩压、脉压、UA、TC、LDL-C、Hs-CRP、sTM呈正相关,均为P<0.01。Logistic回归分析显示,IMT与脉压、LDL-C、Hs-CRP独立相关。结论高Hs-CRP与脉压增大、高LDL-C是老年2型糖尿病患者颈动脉IMT的独立危险因素。Objective To investigate the relationship of high sensitive C reactive protein (Hs-CRP), thrombomodulin (sTM) and other risk factors of atherosclerosis with carotid arterial intima-media thickness (IMT) in elderly patients with type 2 diabetes. Methods A total of 89 elderly patients with type 2 diabetes mellitus were divided into two groups: IMT increased group (IMT 〉 0.9mm, n=61) and IMTnormalgroup (IMT≤0.9, n=28). The levelsofHs-CRP, sTM, blood lipids, fasting plasma glucose (FPG) and other biomarkers were measured and compared between the two groups. Results There were significant differences between ITM increased group and ITM normal group in terms of systolic blood pressure (SBP), pulse pressure (PP), uric acid (UA), low density lipoprotein-cholesterol (LDL-C), Hs-CRP (all P〈0.01), total cholesterol (TC), high densitylipoprotein-cholesterol (HDL-C) and sTM (all P〈0.05). There were no significant differences in diastolic blood pressure (DBP), FPG, fasting insulin (Fins), triglyceride (TG) and glycosylated hemoglobin Alc( HbAlc ) between the two groups. ITM was positively correlated with SBP, PP, UA, TC, LDL-C, Hs-CRP and sTM (all P〈0.01). Logistic regressive analysis showed that the independent risk factors associated with ITM were PP, LDL-C and Hs-CRP. Conclusions High Hs-CRP, PP and LDL-C are independent risk factors associated with ITM in elderly patients with type 2 diabetes.
