摘　　要：目的:探讨MR弥散加权像(DWI)及ADC值在卵巢囊性病变中的诊断价值。材料和方法:对10例巧克力囊肿、10例囊性畸胎瘤、8例附件脓肿、8例囊腺瘤、6例生理性囊肿、20例囊性卵巢癌进行MRI及DWI检查,通过DWI图拟合出ADC图,选取感兴趣区,测量ADC值并进行非参数检验。结果:不同性质卵巢囊性病变ADC值如下:巧克力囊肿(1.01±0.43)×10^(-3)mm^2/s;囊性畸胎瘤(1.80±0.75)×10^(-3)mm^2/s;附件脓肿(1.33±0.75)×10^(-3)mm^2/s;囊腺瘤(2.46±0.34)×10^(-3)mm^2/s;生理性囊肿(2.71±0.07)×10^(-3)mm^2/s;卵巢癌(2.53±0.18)×10^(-3)mm^2/s。卵巢不同囊性病变的ADC值有显著性差异,且良恶性者组间及良性者组内比较均有统计学差异。结论:通过对卵巢囊性病变的ADC值进行定量分析,能为卵巢囊性病变定性诊断提供一定的依据,其中对于巧克力囊肿、囊性畸胎瘤、附件脓肿、生理性囊肿、恶性卵巢癌具有一定的鉴别价值。Purpose： To study the value of MR DWI and ADC value of the ovary cystic lesions. Materials and Methods： 10 cases of endometrial cyst, 10 cases of cystic teratoma, 8 cases of adnexal abscess, 8 cases of cystadenoma, 6 cases of physiology cyst and 20 cases of cystic ovary carcinoma were examined by DWI and conventional MRI, DWI mapping was made to ADC mapping by software, after regions of interests were selected, then the ADC values were measured and nonparametric test was used. Results： The average ADC values of different ovary lesions were as follows： endometrial cyst was（1.01±0.43）×10^-3mm^2/s, cystic teratoma was （1.80±0.75）×10^-3mm^2/s, adnexal abscess was （1.33±0.75）×10^-3mm^2/s, cystadenoma was （2.46±0.34）×10^-3mm^2/s, physiologic cyst was （2.71±0.07）×10^-3mm^2/s, cystic o- vary carcinoma was （2.53±0.18）×10^-3mm^2/s. The ADC values of ovary cystic lesions have significant differences and the ADC value in comparison with benign and malignant groups as well as in benign groups have statistical differences. Conclusion： By analysis of the ADC values for ovary cystic disease, we found that the ADC values can provide qualitation value, and have differential value to endometrial cyst, cystic teratoma, adnexal abscess ,physiologic cyst and cystic ovary carcinoma.
