摘  要:目的:探讨寰枢椎椎弓根或侧块钉棒系统固定用于上颈椎损伤治疗的手术疗效。方法:13例上颈椎损伤患者术前常规行颅骨牵引复位,术中根据术前寰枢椎CT扫描结果决定行椎弓根螺钉或侧块螺钉。结果:术前Frankel分级C级3例、D级5例,术后均恢复至E级。随访6~36个月,所有患者内固定牢固无松动,均获得骨性愈合,临床效果满意。结论:不论寰枢椎椎弓根螺钉还是侧块螺钉钉棒系统固定治疗上颈椎损伤,均具有固定牢固、固定节段短和三维固定的优点。Objective: To observe the effects of atlantosxial transpedicle screw - rod fixation or lateral mass screw for the treatment, of upper cervical injury. Methods: 13 cases with upper cervical injuries were enrolled. Preoperative skull traction was performed. To use the atlantosxial transpedicle screw -rod fixation or lateral mass screw by the CT examination of atlas and axis. The trajectory was adjusted when siguificant resistance was felt during screw processing. Results:3 cases of Frankel grade C and 5 grade D were recovered to grade E. All cases were followed up for 6 ~ 36 months. All gotbony fusion. No instrument looseness was found and the clinical results were satisfactory. Conclusions: The treatment of upper cervical injury with atlautoaxial transpedicle screw - rod fixatiom or lateral mass screw can provide rigid , short - segmental and 3 dimensional fixation.
