摘 要:目的研究快速检测血浆B型钠尿肽(BNP)水平在诊断妊娠期心脏病早期心衰中的作用。方法选取妊娠期心脏病早期心衰患者38例、正常晚期妊娠孕妇35例和正常育龄未孕妇女35例,床边快速检测血浆BNP水平,同时应用彩色多普勒超声测定各组左心室射血分数(LVEF)和心脏指数(CI)。结果早期心衰组和正常妊娠组BNP水平均显著高于对照组(P<0.01),前两组的差异也有显著性(P<0.01)。3组中随着左心室射血分数和心脏指数的下降血液BNP浓度升高,呈良好的负相关。结论BNP检测是妊娠期评价心功能的有效指标,可以快速地帮助诊断妊娠期心脏病早期心衰。Objective To explore the role of rapid assaying plasma level of BNP in diagnosis of early heart failure in pregnancy complicating heart disease. Methods The plasma level of BNP was bedside rapidly detected among 38 pregnant women with early heart failure, 35 normal prgenant women, and 35 normal non-pregnant women. The LVEF and CI were evaluated with Doppler echocardiography. Results The BNP concentration of the normal pregnant women was significantly higher than that of the normal non-pregnant women (P〈0.01), but significantly lower than that of pregnant women with early heart failure(P〈0.01 ). There was significant negative correlation between the BNP concentration with LVEF and CI in three gronps. Conclusion BNP is a valuable biomarker for the evaluation of cardiac function. The assay of plasma level of BNP can be used to diagnose early heart failure rapidly in trimester of prgnancy.
