安徽省晚期血吸虫病医疗负担影响因素分析

摘　　要：目的探讨安徽省晚期血吸虫病人医疗负担的影响因素。方法采用统一的调查表格收集本省2005～2006年晚期血吸虫病外科住院病例的相关资料,应用单因素和多元回归分析方法对264例病人的医疗负担进行分析。结果单因素方差分析结果显示,腹水史、腹水和夹杂症对晚期血吸虫病人医疗负担的影响有统计学意义(P均<0.05)。多元回归分析复相关系数为0.72,决定系数为0.51,模型拟合效果较好,具有统计学意义(P<0.05)。筛选出有统计学意义的影响因素分别为手术情况、术后并发症、输血、年龄组和营养,其相关系数分别为4873.9624、6693.1932、1.9505、42.2733和1104.1666。结论晚血病人自身的年龄、营养状况、腹水史、腹水和夹杂症以及手术情况、术后并发症和术中输血等因素对医疗负担有显著影响。Objective To explore the factors that influence the medical cost of advanced schistosomiasis patients in Anhui Province. Methods The medical records and other related data were collected from 13 pilot hospitals in Anhui Province. Single factor analysis and multiple regression methods on 264 cases of medical cost were used for the analysis. Results The results of single factor analysis showed that the history of ascites, ascites and concomitant diseases for advanced schistosomiasis medical cost had significant differences （P 〈0. 05）. Multiple regression analysis showed the multiple correlation coefficient R was 0. 72, coefficient of determination R^2 =0. 51, and the model fitting better, and there was a statistical significance （P〈0. 05） of the model. The influencing factors with significant differences were surgical process （R = 4 873. 962 4 ）, postoperative complications （R = 6 693. 193 2）, blood transfusion during operation （R= 1. 950 5）, age group （R=42. 273 3）, and nutritional status （R= 1 104. 166 6）. Conclusion Age group, nutritional status, history of ascites, ascites, concomitant diseases, surgical process, postoperative complications, and blood transfusion during operation influence the medical cost of advanced schistosomiasis patients significantly.
