老年心力衰竭426例病因构成分析

摘　　要：目的:对老年心力衰竭(HF)患者的病因构成及多病因特点进行探讨。以期为老年HF的防治提供临床依据。方法:对北京朝阳医院心脏中心HF病房2001年10月～2004年10月收治的426例老年HF患者进行回顾性总结分析。结果:构成老年HF的主要基础病因前三位为冠心病184例(43.2%)、高血压83例(19.5%)及风心病54例(12.7%)。老年HF以多病因HF为多见,共273例(64.1%),其中双病因196例(46%)、三病因64例(15%)及四病因13例(3%)。随年龄的增加,多病因HF比例明显增加。80岁以上100%为多病因HF。结论:构成老年HF的基础病因为冠心病,其次为高血压及风心病。多病因HF在老年HF中起主导地位,为273例(64.1%),其中以双病因HF多见。冠心病、高血压、糖尿病以及慢性阻塞性肺疾患是引起多病因HF常见基础疾病。Objective ：To investigate predisposing cause and outcome of elderly patients with heart failure（HF）. Methods： From October 1. 2001 to October 31. 2004,426 elderly patients hospitalized for HF were included in our Heart Center. The 426 cases were studied retrospectively using the Foxpro HF Date Bank. The underlying diseases and the multiplicity of etiology of HF were analyzed. Results： The first three underlying diseases were coronary heart disease （CHD. 184 cases. 43.2% ） ,essential hypertension （EHT, 83 cases 19.5% ） and rheumatic heart disease（ RHD, 54 cases, 12.7% ） in the elderly patients with CHF. The multiple underlying diseases were most common combinations in the elderly patients with HF. The rate of multiple causes increased with aging, Conclusion： The CHD,EHT and RHD were most common combinations underlying diseases of HF in the elderly inpatients. The multiple causes were chief common combinations in the elderly patients with HF. two multiple causes were most common combinations. CHD, EHT, diabetes, and COPD were most common combinations in multiple causes of HF. The rate of multiple underlying diseases increased with aging.
