抗-HBs阳性HBV感染者随访5年前后生化和超声观察

摘　　要：目的观察抗-HBs阳性HBV感染者五年前后生化、超声影像变化。方法57例抗-HBs阳性HBV感染者根据HBVDNA定量,乙肝病毒血清标志物检测前瞻性随访五年生化,超声影像变化。结果抗-HBs低水平组五年后超声积分增加,门静脉内径增宽,平均流速增加,有统计学差异(P<0.01,P<0.01,P<0.05);HBV DNA高、低载量组五年后超声积分均增高(P<0.01,P<0.05),HBV DNA高载量组五年后球蛋白水平明显增高(P<0.01),门静脉平均流速增加(P<0.05)。结论抗HBs阳性的HBV感染者抗-HBs水平和HBV DNA载量是预后预测指标,抗-HBs愈低,HBV DNA载量愈高,预后愈差。Objective To observe the changes of hepatic function and uhrasonography in HBV - infected cases with anti - HBs positive five years follow - up. Methods Liver function were detected, Quantitative analysis of HBVDNA was performed by polymerase chain reaction, hemodynamics and diameter, liver imaging characteristics were measured using Color Doppler ultrasonography, serological markers of hepatitis B virus were detected by Enzyme - linked immunosorbent assay and ABBOTT in 57 patients with anti - HBs positive during five years follow - up. Results The total indexes of liver uhrasonography、portal vein diameter and velocity increased in low titer anti - HBs groups five years later and differences were significant statistically （ P 〈0.01, 〈0.01, 〈0.05）. The total indexes of liver uhrasonography were higher in high level and low level HBVDNA groups five years later and differences were significant statistically （ P 〈 0.01, 〈 0.05）. Globin and portal vein velocity significantly increased （ P 〈 0.01, 〈 0.05）. Conclusion Titer anti - HBs and level of HBVDNA were predict indexes for HBV - infected eases with anti - HBs positive. The lower titer anti - HBs, the higher level HBVDNA, the poorer prognosis.
