依达拉奉、奥扎格雷和甘露醇联合治疗大面积脑梗死的临床研究

摘　　要：目的评价依达拉奉、奥扎格雷和甘露醇联合治疗急性大面积脑梗死的疗效及安全性。方法2005年8月至2007年5月本院120例急性大面积脑梗死病人,随机分为治疗组和对照组各60例。治疗组应用依达拉奉、奥扎格雷和甘露醇联合治疗;对照组应用奥扎格雷和甘露醇治疗。评价两组治疗前及治疗后7,14,28,90 d的神经功能缺损评分和日常生活能力评分。结果治疗后14 d治疗组神经功能缺损评分和日常生活能力改善均较对照组明显(P<0.05),有效率高于对照组,死亡率明显降低;治疗后90 d,治疗组和对照组神经功能缺损有效率分别为55%和18.3%(P<0.01),日常生活能力有效率分别为51.7%和16.7%(P<0.01)。不增加不良反应。结论依达拉奉、奥扎格雷和甘露醇联合治疗能有效改善急性大面积脑梗死患者神经功能缺损和日常生活能力,未见明显不良反应。OBJECTIVE To evaluate the efficacy and safety of the combined treatment of edaravone, ozagrel and mannitol on acute massive hemispheric infarction. METHODS The cases of massive hemispheric infarction treated in our department from August, 2005 to May ,2007 were divided randomly into treatment group（ n = 60） and control group（ n = 60 ）. The former was treated with edaravone, ozagrel and mannitol, while the latter was treated with ozagrel and mannitol. The scores of the neurological deficits （ESS） and activities of daily living（ADL） were recorded before the treatment and 7,14,28,90 d after the treatment. RESULTS The scores of the neurological deficits （ ESS ） and activities of daily living （ ADL ） were improved in both groups. But the improvement in the treatment group was greater than that in control group （ P 〈 0.05 ） 14d of treatment . The treatment group had a significantly higher effective rate and lower death rate . After 90d of treatment ,the effective rates of the neurological deficits（ESS） in treatment group and control group were 55% andl8.3% respectively （P 〈0.01 ） ,while those of ADL were 51.7% andl6.7% respectively （ P 〈 0.01 ）. The adverse reactions showed no significant difference in two group. CONCLUSION The combined therapy of acute massive hemispheric infarction with edaravone, ozagrel and mannitol can improve effectively neurological deficits and activities of daily living without conspicuous adverse reactions.
