摘 要:目的:观察胺碘酮联合加镁极化液对持续性房颤的转复效果。方法:将符合条件的房颤病人随机分为胺碘酮组(19例)和联合治疗组(20例),胺碘酮组给予胺碘酮150~300mg静脉注射,接着静滴维持24h;联合治疗组胺碘酮的用法与胺碘酮组相同,同时给予500ml加镁极化液静滴,观察24h内的房颤转复情况。结果:两组年龄、性别和房颤持续时间等基本临床特征差异不显著(P>0.05)。两组收缩压和舒张压在治疗前后变化不明显(P均>0.05),两组治疗前后心率明显降低(P均<0.01),但组间比较差异不显著(P>0.05)。恢复窦性心律:胺碘酮组有3例(15.8%)联合组10例(50%),经Pearsonx2检验差异显著(P=0.023,OR=5.33)。Logistic回归分析表明加镁极化液对房颤转复起重要作用(Wald值4.70,P=0.03)。两组均未出现严重不良反应。结论:加镁极化液能提高胺碘酮转复房颤成功率。Objective: To observe the effect of amiodarone combined polarized solution added magnesium sulfate to restore sinus rhythm in patients with persist atrial fibrillation. Methods: Patients were randomly assigned to amiodarone group (n=19) or combination therapy group (n=20). Patients in amiodarone group received 150 mg to 300 mg intravenously followed by intravenous drip over 24 h. Patients in combination therapy group received 500ml polarized solution added magnesium sulfate besides amiodarone. All patients were monitored for 24 h. Results: The difference of age, sex, atrial fibrillation duration and other clinical characteristics between amiodarone group and combination therapy group was not significant (P〉0.05). The change of blood pressure before and after treatment was not significant in both groups (both P〉0. 05 ). Heart rate significantly decreased after treatment in both groups (both P〈0. 01 ), but the difference of heart rate decreased was not significant between two groups (P 〉 0. 05). Three patients (15.8%) in amiodarone group restored to sinus rhythm, and ten patients (50%) restored to sinus rhythm in combination group. Sinus arrhytum conversion rate was higher in combination group (P=0. 023, OR=5.33). Logistic regression analysis indicated that the polarized solution added magnesium sulfate play important role in restoring atrial fibrillation to sinus rhythm (Wald value 4.70, P=0.03). No serve advcrse effects were observed in hoth groups. Con- clusion: Polarized solution added magnesium sulfate can enhance the ability of intravenous amiodarone converted atrial fibrillation.
