超声在股浅静脉瓣膜戴戒术中的应用研究

摘　　要：目的研究术中超声应用于股浅静脉瓣膜戴戒术治疗原发性下肢深静脉功能不全(PDVI)中的疗效和应用价值。方法将经静脉顺行造影证实为原发性下肢深静脉瓣膜功能不全的88个病例分为A、B两组,两组均采用传统的股浅静脉瓣膜戴戒术加曲张静脉剥脱术,A组术中加行超声辅助。以瓣膜反流持续时间评价即时效果,以临床病因-解剖-病理-生理(CEAP)临床分级与临床记分做随访评价疗效。结果术中超声检测静脉反流持续时间戴戒后较戴戒前明显缩短;术后A、B两组临床分级与临床记分差值均有显著性差异(P<0.01)。结论在股浅静脉瓣膜戴戒术中,超声检查进行辅助能够有效提高手术的针对性,增加手术效果。Objective To investigate the value of ultrasonography in encircling constriction of superficial femoral vein operation for the treatment of primary deep venous insufficiency （PDVI） .Methods Eighty- eight patients with PDVI proved by ascending venography were divided into group A and group B. All the-patients in both groups were treated with the encircling constriction. The color Doppler uhrasonography was performed to assist the operations in group A. The lasting time of reflux flow of the veins was used to evaluate the short - term effects, and CEAP classification and clinical scoring were used to evaluate the long - term effects. Results In group A, the lasting time of reflux flow of the veins detected by ultrasonography was shortened greatly after the encircling constriction. The difference of the postoperative CEAP classification and clinical scoring between the two groups was statistically significant （ P 〈 0.01 ）. Conclusion Application of uhrasonography is useful in improving the directions and increasing the effects of the operation of encircling constriction of the superficial femoral vein.
