摘　　要：目的分析改良腭咽成形术(H-UPPP)治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的疗效,探讨围手术期治疗方法。方法对41例患者行H-UPPP术治疗,摘除双侧扁桃体,切除腭帆间隙多余脂肪组织,完整保留悬雍垂,部分重度患者术前行持续正压通气治疗及对症治疗。结果患者术后自觉症状均有明显改善或消失,无误呛及进食返流等症状,术后6个月经多导睡眠呼吸监测(PSG),总有效率82.9%。结论H-UPPP通过保留悬雍垂,维持咽腔基本结构,有效防止腭咽关闭不全,提高手术疗效和安全性。Objective To investigate the curative effect of peri-operative treatment and H-uvulopalatopharyngoplasty（H-UPPP） on obstructive sleep apnea hypopnea syndrome, Methods 41 patients were treated by continuous positive airway pressure for 1 week. Then tonsillectomy and H-UPPP were performed. Result PSG showed the total response rate was 82.9% on the 6th month post-operatively. All signs were improved or disappeared. Conclusion H-UPPP can effectively improve the symptoms of OSAHS and keeps the physical function of uvula and the basic construction of pharynx and prevents velopharyngeal insufficiency.
