细胞间黏附分子-1在家兔急性鼻窦炎上颌窦黏膜中的表达

摘　　要：目的:探讨细胞间黏附分子-1(intercellular adhesion molecule-1,ICAM-1)在家兔急性鼻窦炎上颌窦黏膜中的表达及意义。方法:健康成年新西兰白兔24只,随机分成鼻腔置入Merocel!高分子膨胀海绵+鼻腔注入肺炎链球菌组(A组,8只)、单纯鼻腔置入Merocel!高分子膨胀海绵组(B组,8只)及空白对照组(C组,8只)。10d后将动物处死,完整取出实验侧上颌窦,脱钙3周后采用免疫组化检测上颌窦黏膜中ICAM-1的表达。结果:ICAM-1在A组家兔上颌窦黏膜上皮细胞、腺上皮细胞、血管内皮细胞及固有层炎性细胞中大量表达;B组上颌窦黏膜ICAM-1的表达量也明显增加,但阳性表达强度弱于A组;C组上颌窦黏膜上皮细胞、腺上皮细胞极少量表达ICAM-1,3组间比较差异均有统计学意义(P<0.05)。结论:ICAM-1在实验性急性鼻窦炎鼻窦黏膜中大量表达,在急性鼻窦炎炎症进程中起重要作用。Objective To investigate the expression of intercellular adhesion molecule-1 （ICAM-1） in the maxillary sinus mucosa of experimental acute sinusitis in rabbits and its significance. Methods 24 New Zealand rabbits were randomized to group with nasal cavity placement of Merocel expanded sponge plus injection of Streptococcus pneumoniae （group A,n=8）, group with nasal cavity placement of Merocel sponge alone （group B,n=8）, or blank control group （group C,n=8）. The rabbits were sacrificed ten days after the establishment of the animal model and their maxillary sinus was completely removed. Three weeks after decalcification, the expression of ICAM-1 in the sinus mucosa was detected using immunohistochemitry. Results In group A, ICAM-1 expressed greatly in the mucosal epithelial cells, glandular epithelial cells, vascular endothelial cells, and inflammatory cells in the laminae propria. ICAM-1 expression also enhanced in group B but fewer than that in group A. A scanty of ICAM-1 expressed in the mucosal and glandular cells in group C. There were significant differences among the three groups （P 〈 0.05）. Conclusion ICAM-1 expresses greatly in the maxillary sinus mucosa of experimental acute sinusitis, which may play an important role in the progression of acute sinusitis.
