妊娠高血压患者血管内皮产物变化的临床意义

摘　　要：目的探讨妊娠高血压患者血管内皮产物的变化及其临床意义。方法检测87例妊娠高血压患者的血管内皮产物组织因子抗原(TF-Ag)、纤溶酶原激活物抑制物(PAI)、组织型纤溶酶原激活物(tPA)、血管性血友病因子(vWF)的含量,并与100例健康妊娠妇女及100例健康非妊娠妇女进行比较。结果妊娠高血压患者TF-Ag、PAI、vWF均升高,与健康的晚期妊娠妇女及非妊娠妇女比较差别有统计学意义(P<0.01),且重度子痫前期患者上述指标高于轻度子痫前期和妊娠期高血压患者,差别有统计学意义(P<0.01);妊娠高血压患者和健康妊娠妇女分别与非孕妇相比,tPA均明显升高(P<0.01),但前两者间差别无统计学意义(P>0.05)。结论内皮细胞损伤导致TF-Ag、PAI、tPA、vWF释放,在妊娠高血压的发生、发展中起重要作用,提示血浆TF-Ag、PAI、tPA、vWF水平有可能对妊娠高血压的监测和治疗有一定的临床意义。Objective To evaluate changes and clinical significance of products of hurted endothelium in pregnant women with hypertensive disorder.Methods The levels of tissue factor（TF）,tissue-type plasminogen activator（tPA）,plasminogen activator inhibitor（PAI）,von Willebrand factor（vWF） were determined in 87 pregnant women with hypertension as well as in 100 normal pregnant women and 100 normal nonpregnant women.Results The levels of TF,PAI and vWF were higher in pregnant women with hypertension than in normal pregnant women and nonpregnant women（P〈0.01）.The contents of TF,PAI and vWF in pregnant women with severe pre-eclampsia were significantly higher than those in pregnant women with mild pre-eclampsia and gestational hypertension（P〈0.01）.There were no difference on tPA between the pregnant women with hypertension and the normal pregnant women（P〉0.05）,but the contents of tPA in these two groups were significantly higher than that in normal nonpregnant women（P〈0.01）.Conclusions Injury of vascular endothelium,which leads to the increase of serum TF,PAI,tPA and vWF,plays an important role in occurrence and evolution of hypertensive disorder in pregnant women.Evaluation of products of hurted endothelium has a high value in prevention and treatment of hypertensive disorder in pregnant women.
