摘  要:目的:了解甲氧西林耐药凝固酶阴性葡萄球菌(MRCNS)在医院感染中的分布和流行情况。方法:应用聚合酶链反应检测mecA基因;应用VITEK2检测抗菌药物对MRCNS菌株的最小抑菌浓度;应用脉冲场凝胶电泳(PFGE)对50株MRCNS进行分子同源性检测。结果:MRCNS对环丙沙星、红霉素、左氧氟沙星、诺氟沙星、四环素、复方新诺明的耐药率为48%~80%,对莫西沙星的耐药率为15%~17%,对万古霉素、替考拉宁、利奈唑胺敏感。PFGE结果显示,50株MRCNS中分离自ICU病房的3株溶血葡萄球菌和血液科病房的2株溶血葡萄球菌100%同源,其余菌株则显示出大小不一的同源性或无同源性。结论:MRCNS对多种抗菌药物较高的耐药率应引起临床医师重视;相同病房可增加医院感染传播的机会。Objective:To investigate the prevalence of methicillin-resistant coagulase-negative staphylococci(MRCNS) in the hospital.Methods:MRCNS were detected with mecA-polymerase chain reaction.Minimal inhibitory concentration of MRCNS to antibiotics were determined with VITEK 2.Pulsed field gel electrothoresis(PFGE) was used to analyze the similarities of 50 MRCNS.Results:The resistance rate of MRCNS to ciprofloxacin,erythromycin,levofloxacin, norfloxacin,teracycline and sulfamethoxazole were from 48% to 80% while the resistance rate to moxifloxacin was from 15% to 17%.MRCNS showed susceptive to vancomycin,teicoplanin and linezolid.PFGE results showed 3 S.haemolyticus from ICU ward and 2 S.haemolyticus from haematology ward were 100% similar respectively and others were either of poor similarity or without similarity.Conclusion:It is of great necessity for the clinical physicians to pay attention to the high resistance rate of MRCNS. Same ward could increase the risk of nosocomial infection.No clonal outbreak happened among the 50 MRCNS.
