卡介菌多糖核酸对尖锐湿疣T细胞亚群的调节及疗效分析

摘　　要：目的:研究卡介菌多糖核酸(BCG-PSN)对尖锐湿疣(CA)患者外周血T细胞亚群的影响及临床疗效。方法:对20例正常对照者和60例CA患者(BCG-PSN组40例和病例对照组20例)治疗前后外周血T细胞亚群进行了检测;并对BCG- PSN治疗CA的临床疗效进行了分析。结果:治疗前CA患者外周血CD_3^+T、CD_4^+T细胞百分率和CD_4^+/CD_8^+比值较正常对照组均显著降低(P<0.001);治疗后BCG-PSN组患者外周血CD_3^+T、CD_4^+T细胞百分率于治疗后显著升高(P<0.001),CD^+ T_4/CD_8^+T细胞恢复平衡;病例对照组患者外周血CD_3^+T、CD_4^+T、CD_8^+T细胞百分率治疗前后变化均无统计学差异(P>0.05)。临床观察证实BCG-PSN能够显著提高CA临床治愈率,降低CA的复发率。结论:CA患者存在T细胞亚群失衡;BCG.PSN可使CA患者T细胞亚群恢复正常比例而改善患者的细胞免疫功能,以提高其临床疗效、减少其复发。Objective ： To study the effects of polysaeeharide nucleic acid fraction of bacillus ealmette guerin （BCG-PSN） on T lymphoeytes subsets in patients with candyloma acuminatum （CA）. Method： Flow eytometrie analysis was employed to study T lymphoeytes subsets in peripheral blood of 60 patients with CA （40 patients treated with BCG-PSN, 20 patients treated without BCG-PSN） and 20 healthy controls. Result： Before treatment, the percentage of CO3^＋ T, CO4^＋ T lymphoeytes subsets and the ratio of CD4^＋/CO8^＋ of the CA patients were significantly lower than the healthy control group（ P 〈 0.01 ）. The percentage of CO3^＋ T,CD4^＋ T lymphoeytes subsets , and the ratio of CO4^＋ T/CO8^＋ T returned to normal in CA patients after treatment with BCG-PSN were significantly increased （ P 〈 0. 001 ）. The percentage of CO3^＋ T,CD4^＋ T, CO8^＋ T lymphoeytes were not significantly changed in ease-control group （ P 〉 0.05）. Clinical observations confirmed that the BCG-PSN treatment could significantly increase the clinical cure rate, and decrease the recurrent rate. Condusion： There are T lymphoeytes subsets imbalance in the peripheral blood of CA patients. Treatment with BCG-PSN can recover the imbalance of the ratio of T lymphoeytes subset and improve the eellural immue function in CA patients; the BCG-PSN treatment can significantly increase the clinical cure rate, and decrease the recurrent rate in CA patients.
