降低早产儿脑性瘫痪发生率的临床研究

摘　　要：【目的】探讨早期干预对降低早产儿脑性瘫痪(简称脑瘫)发生率的作用。【方法】将160例早产儿按研究开始前、后的时间和家长意愿分为干预组和常规育儿组,两组患儿自出院后接受常规育儿指导,干预组接受听、视、触觉刺激及水疗、按摩、体操及主动运动训练出现运动异常者作相应康复训练。【结果】6、9、12月龄干预组患儿智力发育指数、运动发育指数明显高于常规育儿组;1岁时干预组患儿大运动、精细动作、语言、个人-社会交往及适应性优于常规育儿组;脑瘫发生率干预组为1.1%,常规育儿组为8.6%,差异有显著性(P值均<0.01)。【结论】早期干预可降低早产儿脑瘫发生率。[Objective] To investigate early intervention to decrease the incidence of cerebral palsy of premature infants. [Methods] 160 premature infants were divided into intervention group and routine care group according to their being studied time and their parents＇ desire.No significant differences were found in sex、average gestational age birth weight,complications of neonatal period,complications of pregnant mothers and Neonatal Behavioral Neurological Assessment Score at 40 weeks of gestational age between the two groups（P〉0.05）.In the intervention group,after premature infants discharged from hospital,they received stimulation of vision,hearing,touch and were given message,subject to exercise and received active motor training in addition to routine care.All infants with abnormal motor manifestations were given appropriate rehabilitation training.In the routine care group,infants received similar routine care only. [Results] When they were at the age of 6,9 and 12 months,all the mental development index（MDI） and psychomotive development index（PDI） of the infants in the intervention group were significantly higher than those of the routine care group（P〈0.01）.At the age of one year,the incidence of cerebral palsy was 1.1% in the intervention group and 8.6% in the routine care group（P〈0.01）. [Conclusion] Early intervention can decrease the incidence of cerebral palsy of premature infants.
