曲美他嗪在老年慢性心力衰竭中的应用

摘　　要：目的探讨曲美他嗪在老年慢性心力衰竭中的治疗效果。方法我院老年科2004年12月至2007年10月资料完整的住院经门诊随访患者40例,在接受原常规药物及药物剂量不变的同时,加用曲美他嗪20mg,每日3次,共12月。治疗前后分别进行临床评估、无创血流动力学监测、血浆BNP水平检测及超声心动图检查。结果曲美他嗪治疗12月后心功能NYHA分级有显著改善(P<0.05)。治疗后心脏指数、每搏输出量、加速度指数、左室射血时间、左室做功指数、左定射血分数较治疗前明显增加,预射血期及血浆脑利钠肽明显减少,差异均有显著统计学意义(P<0.01)。治疗后SBP、DBP、HR较治疗前无明显变化,差异无统计学意义(P>0.05)。结论老年慢性心力衰竭患者在对原来病因等治疗基础上长期应用曲美他嗪可改善心功能,且安全、依从性好、无负性血流动力学影响。Objective To investigate the effect of trimetazidine on elderly paitients with chronic heart failure. Methods Forty elderly patients with chronic heart failure were given 20 mg trimetazidine （3 times per day for 12 months） while they received routine therapy. The clinical evaluation, and the measurement of hemodynamics, echocardiography and the plasma BNP level were performed before and 6 months after therapy. Results After 12 months＇ trimetazidine therapy, there was significant increase in left ventricular ejection fraction （ LVEF）, left ventricular ejection time （ LVET）, cardiac output index （ CI ） , stroke volume （SV） , acceleration index （ ACI ） and left cardiac work index （ LCWI ）, but significant decline in pre-ejection period （PEP） and the plasma BNP level. There was no change in heart rate （HR）, systolic blood pressure （SBP） ,and diastolic blood pressure （DBP） after 12 months＇ trimetazidine therapy. Conclusions Trimetazidine could improve the cardiac function in elderly patients with chronic heart failure after 12 months,but does not affect the he- modynamics.
