摘 要:目的研究带血管蒂筋膜瓣包裹组织工程骨修复兔桡骨缺损对降钙素基因相关肽(CGRP)和神经肽Y(NPY)表达的影响。方法12只新西兰大白兔,每只动物均在左、右侧桡骨中段制作长1.5cm的骨与骨膜缺损,然后植入组织工程骨修复骨缺损,左侧采用带血管蒂筋膜瓣包裹组织工程骨(实验组),右侧仅放置组织工程骨(对照组)。于术后3、6、12个月分别处死4只动物,行免疫组化染色检测修复骨段内CGRP和NPY的表达,并用图像分析软件进行半定量分析。结果两组CGRP和NPY的表达量均随时间延长而增高(P<0.05),且各时间点实验组两者的表达均高于对照组(P<0.05)。3个月时两组CGRP阳性表达部位大致相同,多在血管和骨髓处,新生骨边缘和骨膜处也有少量阳性表达,而6和12个月时以骨膜处和血管周围最多。在各时间点两组的NPY阳性表达部位无明显差异,在灶状骨髓的血管周围表达最多,新生骨质边缘及骨膜处也有少量表达。结论与单纯组织工程骨植入相比,采用带血管蒂筋膜瓣包裹组织工程骨修复兔骨缺损在术后12个月内可以明显促进CGRP和NPY的表达。Objective To evaluate the expressive levels of calcitonin gene related peptide (CASRP) and neuropeptide Y (NPY) in tissue-engineered bone with fascia flap in vivo. Method A segmental bone defect 1.5cm in length was made at the both radius of 12 healthy New Zealand rabbits. The defects were repaired by implantation in two ways: for left radius, engineered bone with fascia flap was implanted (served as experiment group), and for right radius only engineered bone was implanted (served as control group). 3, 6 and 12 months after implantation, the expressions of CGRP and NPY in the new bone were determined with immunohistochemistry and semi-quantified using image analysis software. Results CGRP and NPY expressions in the both groups were significantly increased in a time-dependent manner (P〈0. 05). And the expression in experiment group was significantly higher than that in control group at the end of 3, 6, 12 months (P〈0. 05). In the two groups, CGRP was found mainly in the blood vessels and bone marrow, with a few positive expression at the periphery of newly generated bone and periosteum at the end of 3 months, and then concentrated in periosteum and around blood vessels at the end of 6 and 12 months. The location of NPY in the two groups at the end of 3, 6, 12 months was almost the same, and was mainly around blood vessels in lesion bone marrow with a few expression at the periphery of newly generated bone and periosteum. Conclusions Compared with the control group, implantation with tissue-engineered bone and fascia flap into the bone defects may significantly improve the expressions of both CGRP and NPY during the first postoperative 12 months.
