摘 要:目的与数字减影血管造影(DSA)对比研究,探讨多层螺旋CT血管成像(MSCTA)显示肝动脉解剖变异的应用价值。方法回顾分析138例肝动脉MSCTA和DSA检查资料,对两者肝动脉显影质量进行分级比对;分析评估MSC-TA和DSA显示肝动脉起源、分支走行情况,并根据Michels分型标准进行解剖变异分型。结果MSCTA和DSA两种检查手段对肝叶、肝段动脉显像有很好的一致性,对段以下的肝动脉分支显示,DSA优于MSCTA;综合对比分析,MSCTA和DSA检出正常肝动脉86例,占62.3%;变异肝动脉52例,变异率为37.7%,其中非Micheles分型的少见变异7例,占5.1%。两者显示肝动脉解剖变异的结果无统计学意义。结论与DSA对比研究,MSCTA能对肝动脉解剖变异正确评估,可为肝动脉化疗栓塞治疗和肝脏外科手术提供有价值的信息。Objective To show the applied value of hepatic artery variants by exploring the 16-slice spiral CT angiography(MSCTA),comparing with digital subtraction angiography(DSA)study.Methods According to the degrees of their visualization,assess angiography quality of hepatic artery by analyzing 138 cases MSCTA and DSA.And assess the liver artery origin and its branch.hepatic artery variants were classified according to Michels anatomy.Results The images of lobes of liver and segments of liver of artery made by MSCTA identify well with those by DSA,however,the images of hepatic artery under the segments of liver made by DSA is better than those by MSCTA.Comprehensively analyzed,86 cases were normal,accounted for 62.3%,52 cases were aberrant,accounted for 37.7%,the non-Michels variants are 7,accounted for 5.1%.There is no significant difference between the two groups.Conclusion Comparing with the DSA studies,the MSCTA can do a correct assessment of variant hepatic artery anatomy and provide valuable information for hepatic artery chemotherapy,embolization treatment and liver surgery.
