院前急救模式的对比研究

摘　　要：目的：研究院前急救模式对急救救治效果的影响。方法：2003年1～10月由急诊科兼院前（两栖型）急救出车的1301例病例作为“两栖型”院前急救的对照组；2004年1～10月由专职院前科出诊的1574例作为专业化院前急救的急救组。比较和分析两组间救治效果。结果：急救组和对照组间的平均急救半径，病情分级无显著性差异（P〉0．05）；院内反应时间急救组较对照组明显短（P〈0．01）；院前死亡率急救组较对照组低（P〈0．05）；现场处置率、救治有效率急救组较对照组明显高（P〈0．01）。结论：院前急救模式对患者的救治效果有明显的影响，专业化院前急救救治效果明显优于“两栖型”，专业化院前急救模式也是院前急救发展的方向。Objective： To study the influence of pre-hospital emergency service models on service effectiveness. Methods： The control group included 1 301 cases in which emergence services were delivered by healthcare workers working for both the hospital emergency services and the pre-hospital emergency services （＂ Amphibian＂ model） from January to October 2003. The experimental group included 1 574 cases in which emergency services were delivered by pre-hospital emergency professionals from January. to October 2004. The service effectiveness of these two groups were compared and analyzed. Results： There was no significant difference in the average service radius and the patient＇s condition level between the two groups （ P 〉 0.05 ）. The inner hospital response time in the experimental group was significantly shorter than that in the control group （P 〈0.01 ）. The pre-hospital mortality was lower in the experimental group than that in the control group （ P 〈 0.05 ）. The local treatment rate and the service effectiveness rate in the experimental group was significantly higher than that in the control group （P 〈0.01 ）. Conclusion： Pre-hospital emergency service models can significantly influence service effectiveness. The service effectiveness of professional pre-hospital emergency service model is significantly better than that of ＂Amphibian＂ model. Professionalism should be the direction of pre-hospital emergency services.
