摘 要:目的:探讨牵引闭合复位结合动力髋(DHS)内固定治疗股骨粗隆间骨折的临床效果。方法:本组32例患者均在C臂X线机透视下利用牵引床进行闭合复位并且行切开DHS内固定,常规术后抗炎治疗及护理。术后早期功能锻炼。结果:本组共治32例,随访13~24个月,平均19个月,所有患者均获得临床骨性愈合,髋内翻畸形1例,无断钉等髋关节严重活动障碍,按Harris疗效评定标准,术后髋关节功能评价优良率93.7%。结论:牵引闭合复位结合DHS内固定治疗股骨粗隆间骨折疗效可靠,是一种有效的治疗方法。Objective: To observe the therapeutic effect of closed traction reduction and dynamic hip screw (DHS) for treatment of femoral intertrochanteric fracture. Methods: Thirty-two patients were treated by closed traction reduction and open internal fixation by DHS in traction bed under C-arm X-ray machine guidance. After operation, routine anti-inflammatory treatment, nursing, and early functional exercise were employed. Results: A 13- to 24-month follow-up (average: 19 months) of the 32 patients revealed satisfactory clinical bone union in all patients, only 1 case of coax vara deformity, and no screw breakage and joint dysfunction. According to Harris criterion, the postoperative fineness rate of the functional assessment reached as high as 93.7%. Conclusion: Closed traction reduction and DHS is an effective method for management of femoral intertrochanteric fracture.
