摘  要:①目的探讨白细胞粘附功能(LAF)和细胞粘附分子(CAMs)表达与肺心病的关系。②方法采用酶联免疫吸附法(ELISA),分别检测了68例肺心病病人急性加重期和缓解期及46例健康人外周血白细胞CD18表达,血清可溶性细胞间粘附分子-1(sICAM-1)和白细胞粘附率(LAR)的变化。③结果肺心病病人白细胞LAF,CD18表达及sICAM-1浓度明显增高,与对照组比较差异有极显著性(t=6.902,21.482,31.923,P均<0.001),且急性加重期较缓解期增高更明显(t=4.578,8.264,17.806,P均<0.001)。肺心病病人白细胞LAF与CD18表达及sICAM-1浓度呈正相关(r=0.672,0.803,P均<0.001)。④结论LAF增高和CAMs表达增加参与了肺心病的发生与发展,以及与病情严重程度均有密切关系。Objective To study the relationship between the leukocyte adhesion function(LAF), cell adhesion molecules(cAMS) expression and cor pulmonale. Methods The changes of leukocyte CD 18 expression, serum soluble intercellular molecule 1(sICAM 1) and leukocyte adhesion rate(LAR) were measured in 68 patients with cor pulmonale during the acute attack and remission stages and 46 controls by using ELISA, thrombus and platelet adhesion dual purpose apparatus. Results The LAR, expression of leukocyte CD 18 and sICAM 1 concentration were significantly higher in patients than those in controls( t= 6.902, 21.482, 31.923, P <0.001), and these indicators were more obvious in acute attack stage than those in remission stages( t=4.578, 8.264, 17.806, P<0.001 ). In cor pulmonale patients, the LAF was positively correlated with leukocyte CD 18 expression and sICAM 1 concentration( r=0.672, 0.803, P <0.001). Conclusion The increase of LAF and expression of cAMS may participate in the occurrence and development of cor pulmonale and these changes are closely related with the seriousness of patients.
