摘 要:目的探讨常温(25℃)下标本放置时间对凝血酶原时间、活化部分凝血活酶时间、纤维蛋白原(凝血三项)测定结果的影响。方法采血后2 h凝血三项测定值分别与采血后4 h、6 h、8 h测定值进行比较,计算两时间段间测定结果的相关系数,作线性回归,求回归方程;把医学决定水平X1、X2、X3分别代入回归方程求Y值,计算各给定的医学决定水平的系统误差,系统误差小于CLIA 88规定的室间质量评价的总允许误差1/2作为判断标准,以医学决定水平处的系统误差来判断两时间段间测定结果一致性是否为临床可接受。结果凝血酶原时间、纤维蛋白原采血后2 h与采血后4 h、6 h、8 h检测结果具有一致性,为临床可接受;活化部分凝血活酶时间采血后2 h与采血后4 h、6 h检测结果具有一致性,为临床可接受,与采血后8 h检测结果为部分接受。结论凝血酶原时间、纤维蛋白原检测时间,在常温(25℃)下原始标本可稳定8 h,而活化部分凝血活酶时间最好在采血后6 h内测定。Objective To study the influence of preservation time of samples on measured values of prothrombin time(PT),activated partial tromboplastin time(APTT) and fibrinogen(FIB) at room temperature. Methods The values measured at 2 hours storage time were compared to that at 4, 6 and 8 hours respectively. The correlation coefficient was calculated in measured values between two different storage time, then regression equation was made by using linear regression. The medical determination level X1, X2 and X3 were imported to regression equation for calculating Y value. The systemic error(SE) was calculated from given medical determination level, and the clinical acceptability of the consistency was assessed between different measuring times according to the standard of CLIA 88 in which the level of SE was in half of total allowable error in external quality assessment. Results The measured values of PT and FIB were accepted when samples were stored for 2, 4, 6 and 8 hours because of the consistence of the values measured at different storage time. The value of APTT was also accepted clinically as the consistence of measured results between samples stored for 2, 4 and 6 hours. However, the value of APTT was partly accepted because of the partly consistence of measurement between samples stored for 2 and 8 hours. Conclusions The measured values of PT, FIB are stable for 8 hours while that of APTT is stable only for 6 hours when samples are kept at room temperature (25℃). So it would be better if APTT is measured within 6 hours.
