对称单蒂复合乳腺组织瓣矫正乳头内陷对哺乳的影响

摘　　要：目的:探讨对称单蒂真皮乳腺组织复合瓣乳头凹陷矫正术对哺乳功能的影响。方法:收治37例61个乳头凹陷患者,采用以乳晕边缘为蒂、对称的两个单蒂真皮乳腺复合组织瓣矫正乳头内陷,重塑乳头。结果:对61个乳头外形随访3～6个月,3个乳头高度欠佳,有不同程度回缩,其余乳头高度及外形满意。对哺乳功能随访18个月,随访到已生育者50例,其中,乳头中度凹陷矫正者43例,重度凹陷矫正者7例。中度凹陷矫正后可哺乳者41例,占95%(41/43);重度凹陷矫正后可哺乳者5例,占71%(5/7);不能正常哺乳者,中、重度各2例。结论:对称单蒂真皮乳腺组织复合瓣是矫正乳头凹陷的一种较好的手术方法,创伤小,能保留哺乳功能。Objective To discuss the effection on the breast feeding after using the symmetry single pedicle derma and breast complex flap to correct the inverted nipple. Methods Using the areola papillary edge as the pedicle, correct the inverted nipple with the symmetry single pedicle derma and breast complex flap. Results Sixty-one received 3 to 6 months followed-up after operation, only 3 nipples seemed imbalance in height and shrank a little. The other had a satisfactory height and physical form. With 18 months followed-up after operation, 50 cases could be breast-fed, in which 43 cases were severe inverted patients and 7 cases were moderate ones. Of moderate inverted patients, 41 cases could be breast-fed after operation. The rate was 95% （41/43）. Of the severe inverted patient, 5 cases could be breast-fed after operation, The rate was 71% （5/7）. Only 2 cases separately on moderate and severe patients could not be breast-fed. Conclusion The symmetrical single pedicle derma and breast complex flap is an effective and microinvasive method to correct the inverted nipples.
