摘  要:目的了解江门地区部分医院下呼吸道感染病原菌分布及对抗菌药物的耐药情况。方法用回顾性调查的方法对培养分离的1 591株病原菌,从种类、药物敏感性方面进行汇总和分析。结果以革兰阴性杆菌为主,占62.7%;其中铜绿假单胞菌、肺炎克雷伯菌、大肠埃希菌分列前3位;超广谱β-内酰胺酶(extendspectrumβ-lactamases,ESBLs)表型确证试验ESBL检出率:大肠埃希菌为53.4%(62/116),肺炎克雷伯菌为30.6%(48/157)。革兰阳性球菌占16.7%,耐甲氧西林金色葡萄球菌(Methicillin-resistant S.aureus,MRSA)和耐甲氧西林凝固酶阴性葡萄球菌(Methicillin-resistant coagulase-negative Staphylococci,MRCNS)检出率分别为72.0%(90/125)和82.5%(80/97);未发现耐万古霉素葡萄球菌。真菌占20.6%。结论本次研究中肠杆科细菌产ESBL水平、非发酵菌的耐药率和耐甲氧西林葡萄球菌的检出率均较高。临床医师应根据药敏结果正确合理地使用抗生素。Objective To investigate the characteristics and drug resistance of bacteria of the lower respiratory tract specimens obtained from some hospitals of Jiangmen. Method The distribution of 1591 bacterial strains and antimicrobial susceptibility of these strains were analyzed retrospectively. Result Gram-negative bacilli occupied 62.7%. The 3 common gram-negative bacilli were Pseudomonas aeruginosa, Klebsierlla pneumoniae and Escherichia coli. ESBLs were identified in 53.4%(62/116) of Escherichia coli isolates, and in 30.6%(48/157) of Klebsiella pneumomae isolates. Gram-positive coccus occupied 16.7%. The detection rates of MRSA and MRCNS were 72.0%(62/116) and 82.5%(80/97), respectively. Vancomycin-resistant Straphylococcus aureus was not identified in this study. Fungi accounted for 20.6%. Conclusion The production rate of ESBLs, multi-drug resistance of non-fermenters and methecillin resistance of staphylococcus in local region were high. Clinicians should apply antibiotics rationally in order to reduce the drug resistant bacterial strains.
