改良真皮帽双环法巨乳缩小整形术

摘　　要：目的探讨一种能够保持乳房功能和形成良好外观,并发症较少的乳房缩小整形术。方法采用以乳晕头中心的双环切口,去除表皮,采用内上腺体蒂技术,切除以外上象限为主的乳腺组织,将保留的乳腺组织重新塑形。结果采用此法手术治疗13例26侧,乳房术后形态良好,无并发症,乳头乳晕感觉良好,效果满意。结论本术式是一种安全、有效的手术方法,能获得持久的塑形效果,通过选择不同类型的皮肤切口可以适用于各种程度乳房肥大的矫治。Objective To explore a technique, which keeps good configuration and function after reduction mammoplasty, and without little complications. Methods Two concentric circles were designed with the nipple in the centre, the skin between the two circles was de-epithelized, then the wedge-shaped section of glandular tissue in lateral and superior was excised, the superomedial glandular pediele technique was applied, The remaining superomedial glandular tissue was remodeled as a conical shape. Results Thirteen patients （26 breasts） of operation have been performed successfully. The results were satisfactory and there were no complications. The breasts maintained a lasting and aestheticallypleasing shape and the nipple areola preserved a good sensation. Conclusions This method is worthy to be applied widely because the good result with effective safety and reliability is a safe and effective technique which can provide long lasting outcome. The design of the procedure can be adapted to a variety of skin incision patterns and breast morphologies
