自固化磷酸钙糊剂与氢氧化钙修复乳磨牙髓室底穿孔的材料性能比较

摘  要:对比观察自固化磷酸钙糊剂与氢氧化钙糊剂修复乳磨牙髓底穿孔效果。选择乳磨牙髓底穿孔病例95例106颗牙,按随机数字表法分为氢氧化钙组和自固化磷酸钙组,每组53颗患牙。经根管预备和碘伏髓腔消毒后3~5d,自固化磷酸钙组用注射式糊剂对根管和髓底穿孔处充填封闭,氧化锌粘固粉暂封1周,无症状后做永久性封闭;氢氧化钙组用氢氧化钙1份,氧化锌1份,碘仿少许与丁香油调成糊剂,将该材料置于穿底处后,常规根管充填,再用氧化锌粘固粉暂封,观察1周,无症状后去除部分氧化锌,作永久性封闭。结果表明,自固化磷酸钙组治疗髓底穿孔的成功率高于氢氧化钙组。术后0.5,1,1.5年自固化磷酸钙组成功率分别为92.45%,90%及86.67%;氢氧化钙组成功率分别为77.36%、70.21%及68.18%。两组间差异有显著意义(P〈0.05)。可见采用自固化磷酸钙治疗乳磨牙髓底穿孔较氢氧化钙优越,是较为理想的髓底修复材料。To compare the effect of autosolidifying calcium phosphate cement and calcium hydroxide in the repair of perforation in pulp chamber floor of deciduous molars. Ninety-five patients with deciduous molars, which had perforated in pulp chamber floor, involving 106 teeth, were randomly divided into two groups, each group containing 53 teeth. Three to five days before treatment, root Canal preparation and medullary cavity disinfection with iodophor were performed. In group one, the autosolidifying calcium phosphate cement injectable paste was used for root canal filling, and the perforation in pulp chamber floor was temporarily closed using zinc oxide cement for one week, followed by permanent closure if no symptoms were found. In group two, a portion of calcium hydroxide, a portion of zinc oxide, a little iodoform and eugenol were prepared into paste. The prepared material was placed under the perforated site prior to the conventional root canal filling. Zinc oxide cement was then adopted to perform temporary closure and permanent closure after the removal of zinc oxide, indicated by asymptomatic observation for one week. The result of this experiment showed that, the success rate of autosolidifying calcium phosphate cement treatment was significantly higher than that of calcium hydroxide treatment (P 〈 0.05). At 0.5, 1 and 1.5 years postoperatively, the successful rates were 92.45%, 90% and 86.67% in group one, while 77.36%, 70.21% and 68.18% in group two. Obviously, autosolidifying calcium phosphate cement treatment is superior to calcium hydroxide treatment for the perforation in pulp chamber floor of deciduous molars, so it is an ideal material of repairing myeloid perforation.
