摘 要:背景:近年永磁性材料固位体已较多地用于上下颌多数牙缺失的固位中,可显著提高义齿的固位力,但用于重度牙周病与牙周组织的生物相容性还有待于进一步观察。目的:观察重度牙周病全口覆盖义齿磁性材料固位体基牙,修复牙周组织1年后的组织相容性的状况。设计、时间及地点:对比病例观察,于2007—01/2008—01在中国医科大学口腔医学院和民航总医院完成。对象:重度牙周病伴牙列缺损患者18例。采用日本生产的Magfit Ex400磁性固位体和注射型寒天琼脂印模材;美国生产的齿科钴铬合金。方法:患者上颌或下颌仅余留1~4个磨牙,选择1~2颗基牙安置磁性固位体,余牙作为覆盖基牙,共选择基牙32颗。患者先进行根管治疗,再经1~2个月的牙周治疗,使患牙情况相对稳定后行修复治疗。主要观察指标:牙周治疗后3个月及磁性固位体修复后12个月观测基牙的菌斑指数、牙龈指数、牙周袋探诊深度、附着丧失的变化。结果:磁性固位体修复后12个月菌斑指数和牙龈指数均低于牙周治疗后3个月,差异有显著性意义(P〈0.05),牙周袋探诊深度和附着丧失比较差异无显著性意义(P〉0.05)。18例患者中1例义齿在磁性固位体设置处折裂,4例覆盖基牙龈缘处有炎症。无继发龋发生。结论:重度牙周病伴牙列缺损患者全口覆盖义齿可以使用磁性固位体进行固位,1年后牙周组织状态良好,未发生生物相容性不良反应。BACKGROUND: Magnetic attachments have been frequently used as the retainers in treatment of the odontoptosis in upper and lower mandible at recent years. It can significantly raise the retention of the denture, but more researches are required to verify its biocompatibility on serious periodontal disease. OBJECTIVE: To investigate the periodontal tissues on serious periodontal diseases repaired with magnetic attachments in complete overdenture. DESIGN, TIME AND SETTING: A control case observation was carried out in the College of Stomatology, China Medical University and the General Hospital of Civil Aviation Administration of China from January 2007 to January 2008. MATERIALS: Eighteen patients of serious periodontal disease and dentition defect were recruited for this study. Magfit EX400 magnetic retention apparatus and injectable agar modeling composition were produced by Tantian (Japan); Dental Co-Cr alloys were from America. METHODS: Only 1-4 molars were remnant in each patient, either upper mandible or lower mandible. Magnetic retention was performed in 1- 2 molars, while other 32 teeth served as magnetic attachments. Subsequent to root canal therapy and periodontal therapy, the patients received prosthodontic treatment. MAIN OUTCOME MEASURES: The changes of plaque index, gingival index, periodontal pocket depth and attachment loss were determined 3 months after periodontal therapy and 12 months after overdenture restoration in 18 patients. RESULTS: Following the overdenture restoration in 18 patients, the plaque index and gingival index were significantly lower than those 3 months after periodontal therapy (P 〈 0.05). There were no significant differences in the periodontal pocket depth and attachment loss (P 〉 0.05). Denture breakage occurred in one case, and inflammatory reaction was noted in four cases.
