摘  要:目的:建立一种快速、廉价、特异性高、准确检测强直性脊柱炎(AS)患者人类白细胞抗原B27(HLA-B27)的方法。方法:选择人类白细胞抗原B27蛋白基因组保守区域设计上游引物,下游引物,优化PCR反应体系。对450例AS疑似标本和50例健康体检标本同时采用流式细胞技术(FCM)检测对照其结果进行方法学评价。结果:(1)成功的构建了PCR反应体系。(2)450例AS疑似标本聚合酶链反应(PCR)法检出HLA-B27阳性247例,阳性率54.89%,流式细胞技术检出HLA-B27阳性250例,阳性率55.56%,50例健康体检标本HLA-B27两种检测方法均为阴性。两者复合率达98%以上,对20例阳性标本进行多次PCR实验,重复性一致,未见批间和批内差异。(3)初步临床应用证实该方法结果准确、灵敏度高。结论:采用聚合酶链反应检测HLA-B27的方法灵敏度高、特异性强、操作简便、廉价、快速,更适合临床检验诊断的要求。Objective:To establish a fast, inexpensive, highly specific and accurat system to be used in ankylosing spondylitis (AS) patients with human leukocyte antigen B27 (HLA - B27 ) method. Methods:Human leukocyte antigen B27 genome conservative region upstream primer design and downstream primers, optimizing PCR system. As the 450 cases and 50 cases were suspected healthy specimens at the same time by flow cytometry (FCM) results of the control method of evaluation. Results: ( 1 ) Construction of the success of the PCR reaction system. ( 2 ) AS 450 cases of suspected specimens polymerase chain reaction (PCR) were HLA - B27 -positive rate of 247 cases were 54. 89% , flow cytometry were HLA -B27 -positive rate of 250 cases were 55.56%, and 50 cases of healthy Two samples of HLA - B27 detection method were negative. Taken compound rate of more than 98% of the 20 cases of a number of PCR positive samples were consistent experimental repeatability, and no intra and inter difference. (3) preliminary clinical application of this method proved accurate, high sensitivity. Conclusion:Detected by polymerase chain reaction method of HLA - B27 high sensitivity, specificity, simple, cheap, faster and more suited to the requirements of clinical laboratory diagnosis.
