43例重度骨盆骨折合并多发伤的救治分析

摘　　要：目的探讨重度骨盆骨折合并多发伤的早期临床特点,研究严重骨盆骨折的诊治策略。方法回顾性分析2005-01～2008-03收治的43例骨盆骨折合并不同类型多发损伤患者的诊断、治疗经过,总结其伤情及抢救要点。结果经早期诊断和心肺复苏,多学科协同治疗,手术治疗,治愈42例,死亡1例。结论对重度骨盆骨折合并多发性创伤患者,应重视颅脑、胸腹脏器、泌尿生殖系等合并伤的诊断,有效控制大出血、多学科协同治疗和恢复骨盆环的完整性是治疗严重骨盆骨折合并多发伤的关键。Objective To investigate the early clinical characteristics,diagnosis and emergency treatment of severe pelvic fracture combined with multiple trauma.Methods We studied retrospectively the diagnosis and treatment of the 43 cases with pelvic fracture combined with multiple trauma of different types from January 2005 to March 2008,and summarized the wound condition and experience of emergency treatment.Results By early diagnosis and effective resuscitation,cooperative treatment of many departments and operations,42 patients were cured and 1 patient died.Conclusion Diagnosis of compound injury including encephalon,celiac organs or genitourinary system,effective control of hemorrhage,cooperative treatment of many departments and restoration of the integrity of the pelvic ring are the key to improve the survival rate of patients of severe pelvic fracture combined with multiple trauma.
