摘　　要：目的:观察白细胞介素(IL)-4对重型肝炎患者外周血单个核细胞(PBMCs)肿瘤死因子-α(TNF-α)表达的影响,以了解 IL-4在重型肝炎治疗中的潜在应用价值。方法:应用 Logo 细胞毒试验、免疫组织化学法和半定量逆转录-多聚酶链反应(RT-PCR)检测 TNF-α表达水平。结果:IL-4在蛋白质和 mRNA 水平明显抑制脂多糖(LPS)诱导的亚急性和慢性重型肝炎患者 PBMCs TNF-α的产生,这种抑制作用呈剂量和时间依赖的方式。IL-4于100U/ml 时开始出现抑制效应,于1000U/ml 时抑制作用接近最大。虽然 IL-4对慢性重型肝炎 PBMCs TNF-α的抑制作用略低,但与未经 IL-4处理的 PBMCs 相比较,TNF-α的表达水平仍有显著下降。结论:IL-4是一种有效的 TNF-α下向调节因子,在重型肝炎的治疗上具有潜在的应用价值。In order to observe the effect of IL-4 on expression of TNF-α by peripheral blood mononuclear cells (PBMCs) from patients with severe viral hepatitis and estimate the potential clini- cal value of IL-4 in the treatment of severe hepatitis.Methods:The expression of TNF-α by PBMCs was detected by L_(929) cell cytotoxic assay,immunohistochemical assay and semiquantitative reverse transcriptional polymerase chain reaction(RT-PCR).Results:When the isolated PBMCs were cultured in the presence of increasing concentration of IL-4 or IL-4 was added to cultures at various intervals after exposure to lipopolysacchride (LPS),it was found that IL-4 in dose- and time-dependent manner downregulated the LPS-stimulated production of TNF-α by PBMCs from patients with severe hepatitis at protein and mRNA levels.The inhibitory effect of IL-4 could be observed at a concentration of 100U/ml and reached maxi- mum at 1 000U/ml.Although the suppression action of IL-4 on the production of TNF-α by PBMCs from chronic severe hepatitis was slightly lower than that from subfulminant hepatitis,there existed significant difference in these two type of severe hepatitis as compared with the control PBMCs untreated with IL- 4.Conclusion:IL-4 seems to be a potent down-regulator of TNF-α and may be potential in the treat- ment of the severe viral hepatitis.
