青壮年肺血栓栓塞症的临床特点及病因分析

摘　　要：目的探讨分析青壮年肺血栓栓塞症(PTE)患者的临床特点及发病原因。方法回顾性分析45例确诊住院PTE患者临床资料。结果(1)青壮年患者(<60岁)24例(24/45,53.35%),平均年龄(35.0±7.9)岁;老年患者(>60岁)21例(21/45,44.4%),平均年龄(73.0±7.5)岁。(2)青壮年组危险因素依次为下肢静脉血栓(DVT)、手术史、心脑血管病史、骨折、肿瘤,老年组危险因素依次为DVT、心脑血管病史、手术史、肿瘤、骨折;心脑血管病史在青壮年组比老年组低(5/24∶10/21),差异有显著性(P<0.05)。(3)青壮年和老年组PTE的主要临床表现为呼吸困难、咳嗽和心悸,但均无特异性。(4)老年组死亡率较高(1/21∶0/24),差异有显著性(P<0.05)。结论青壮年PTE患者发病率高,临床表现无特异性,危险因素多。Objective To evaluate the clinical characteristics and etiology of young-middle-age patients with pulmonary thromboembolism （PTE） .Methods The clinical data of 45 patients with PTE were analyzed retrospectively.Results （1）There were 24 young-middle-age patients （〈60 years of age） with PTE （24/45,53.35%） whose mean age is 35.0 ± 7.9, and 21 elderly patients （21/45, 44.4%） whose mean age is 73.0 ± 7.5 ; （2） Risk factors in young-middle-age group were lower limb venous thrombosis （DVT） ,operation history, cardiovascular and cerebrovascular disease history, as well as fracture. While the risk factors in elderly group were DVT,cardiovescular and cerebrovascular disease history, operation history, tumor and fracture in order. The rates of cardiovascular and cerebrovascular disease history were lower （5/24 ： 10/21） in young-middle-age group than in elderly group （P〈0.05） ; （3） The main clinical manifestations of PIE in young-middle-age and elderly people were dyspnea, cough and palpitation. But all symptoms were nonspecific; （4） The mortality of PTE was higher in elderly group than in young group （1/21 ： 0/24）（P〈 0.05） .Conclusion The morbidity of PTE in young patients is higher than the elder, but the clinical manifestations are nonspecific.
