超选择性肾动脉栓塞治疗肾脏损伤出血的探讨

摘　　要：目的探讨超选择性肾动脉栓塞治疗肾脏损伤出血的应用价值。方法12例各种原因引起的肾脏损伤出血患者在数字减影血管造影下,采用明胶海绵或(和)钢丝圈进行超选择性肾动脉栓塞治疗。结果所有患者术后3～5d肉眼血尿消失,平均随访10个月,无肾出血复发,血尿素氮、肌酐及血压正常,肾功能正常。结论超选择性肾动脉栓塞术创伤小、疗效好、并发症少,能最大限度保护病肾功能,是治疗肾脏损伤出血的安全有效方法。Objective To discuss the application of transcatheter super-selective arterial embolization on treatment of renal traumatic hemorrhage. Methods 12 patients underwent digital subtraction angiography with definite diagnosis of traumatic renal hemorrhage before embolization. Materials used in the embolization were gelfoam sponge chips and steel coils through super-selective renal arterial embolization. Results Macroscopic hematuria disappeared on 3rd to 5th day after operation. With an average follow-up of 10 months, no hemorrhage reoccurred in renal, renal function and blood pressure were normal in all patients. Conclusion Super-selective renal arterial embolization has advantages of less invasive,less complications,and good haemostatic efficacy, with maximal preservation of renal function for treatment of renal injuries.
