摘 要:目的:探讨镫骨与砧骨长突的连接方式对声音传导的影响,以指导镫骨手术。方法:根据复旦大学附属中山医院提供的人体正常耳CT扫描结果,用自编软件将CT扫描数值化并导入PATRAN重建三维有限元模型,应用动力学的传导振动原理,对正常的听小骨结构进行动力数值计算分析,并与目前已报道的试验结果对比。对采用环型置换术式,用不同环直径的镫骨赝附体所重建的听骨链结构进行动力数值计算分析,并与正常的听骨链结构力学行为进行对比。结果:在环型镫骨置换术中,镫骨赝附体与砧骨长突连接处环直径的大小对镫骨的振幅有影响,即当置换环直径越小时,镫骨赝附体振幅越接近正常人体,患者听力恢复效果越佳。结论:镫骨赝附体与砧骨长突连接处环直径的大小对患者术后的听力恢复有影响。Objective:To evaluate the ring diameter of junction between stapes protheses and incus long process on sound transmission in the middle ear after stapes replacement,and to use this information to guide stapes surgery.Methods:The CT data of ossicle from Zhongshan Hospital of Fudan University,were digitalized and imported into PATRAN(a world class commercial software.) to build a 3 dimension finite element model(FEM).Then the ossicle model was analyzed based on dynamic transmitting vibration principles and the calculation results were evaluated and compared with results from examination.Using stapes protheses of different ring diameter to reconstruct Ossicle chain in stapes replacement and analyzed based on dynamic transmitting vibration principles,the calculation results were evaluated and compared with natural Ossicle chain.Results:The ring diameter of junction between stapes protheses and incus long process can effect the amplitude of protheses in ring stapes replacement.The smaller the ring diameter,the amplitude of protheses is closer to the normal,and the hearing recovering effect will be better.Conclusion:The ring diameter of junction between stapes protheses and incus long process can effect the hearing recovering after stapes replacement.
