七味白术散合补阳还五汤治疗肥胖2型糖尿病及改善胰岛素抵抗的研究

摘　　要：目的观察七味白术散合补阳还五汤加减治疗肥胖2型糖尿病及改善胰岛素抵抗的作用。方法将64例肥胖2型糖尿病胰岛素抵抗患者随机分为2组,2组基础治疗相同,治疗组加用中药饮片(由七味白术散合补阳还五汤加减组成),15 d为1个疗程,观察3个疗程,主要观察治疗前后空腹血糖(FBG)、餐后2 h血糖(2 h PG)、空腹胰岛素(FI NS)、血清胆固醇(TC)、三酰甘油(TG)、体质量指数(BMI)、胰岛素敏感指数(ISI)等指标的变化。结果治疗组总有效率(88%)明显高于对照组(62%)(P<0.05)。治疗后治疗组FBG、2 h PG、TC、TG、BMI、FI NS、ISI均明显改善(P均<0.01),对照组仅FBG、2h PG、TC有明显改善(P均<0.05)。治疗后2组BMI、2 h PG、TC、TG、FI NS、ISI比较均有显著性差异(P均<0.05)。结论在基础治疗的同时加用七味白术散合补阳还五汤加减治疗肥胖2型糖尿病,能改善临床症状,降低BMI,改善糖、脂代谢,并且能显著增加胰岛素敏感性,疗效明显优于单用西药。Objective It is to observe the function of Qiwei Baizhu powder combined with Buyang Huanwu decoction on type 2 diabetes mellitus with obesity and insulin resistance. Methods 64 patients with type 2 diabetes mellitus with obesity and insulin resistance were randomly divided into 2 groups. The basic therapy in both groups was identical. Treatment group was added with traditional Chinese medicine including Qiwei Baizhu powder and Buyang Huanwu decoction. 15 days was one therapy course. The patients were treated for 3 courses. The changes of indexes including FBG, 2 h PG, FINS, TC, TG BMI and ISI were observed. Results The total effective rate（88 % ） was significantly higher than that（62 % ） in control group （P〈 0.05）. All the indexes above in treatment group were improved obviously after therapy（P〈 0.01）, but only FBG, 2 h PG and TC improved in control group （P〈0.05）. There were significant differences in BMI, 2 h PG, TC, TG, FINS and ISI between the two groups（ P 〈 0.05 ）. Conclusion Therapy of Qiwei Baizhu powder and Buyang Huanwu decoction basis on basic treatment can improve clinical symptoms and metabolism of carbohydrate and lipoid, low down BMI, and significantly increase sensitivity for insulin. Its curative effect is better than simple western medicine.
