摘 要:目的探讨成功施行腹腔镜脾切除术(Laparoscopic Splenectomy,LS)的技巧。方法对32例LS患者的临床资料进行回顾性分析。结果32例中31例手术成功,中转开腹1例。手术时间平均95 min,术中平均出血量220 ml。术后12 h拔胃管,36 h开始进食流质。术后平均住院6 d,并发症2例,发生率6.45%,其中左膈下积液1例,切口感染1例。结论合适的病人选择,正确的体位和良好的显露,可靠的脾蒂处理方法是保证LS成功的重要因素。Objective To investigate the skills of laparoscopic splenectomy(LS). Methods The clinical data of 32 patients with LS were retrospectively analyzed. Results 31 patients with Laparoscopic splenectomy were performed successfully. Because of hemorrhage of the splenic pedlcle, 1 patient needed laparotomy to complete the operation. The average period of operation was 95 min and the average of blood loss was 220ml. Postoperative complications occurred in 2 cases, including incision infection and infradiaphragm fluid collection. Conclusions Strict selection of patients, suitable position and satisfactory exposure and reliable control of the splenic pedicle are the keys of LS.
