无创通气治疗COPD合并Ⅱ型呼吸衰竭疗效观察

摘　　要：目的观察双水平正压通气(BiPAP)治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床疗效。方法选择我院近年来收治的COPD合并Ⅱ型呼吸衰竭患者48例,随机分为治疗组和对照组各24例,两组均给予常规基础治疗,治疗组在此基础上给予BiPAP呼吸机进行无创机械通气治疗,对照组给予双鼻导管吸氧治疗。结果两组患者治疗后临床症状均有不同程度缓解,治疗组较对照组起效更快,疗效更好;对照组住院时间22.5±3.2 d,治疗组住院时间11.6±2.8 d;两组治疗后血气分析均有显著改善,治疗组改善程度明显优于对照组,两组疗效比较差异有统计学意义(P<0.05)。结论BiPAP无创通气治疗COPD合并Ⅱ型呼吸衰竭无创伤、疗效确切,能迅速纠正缺氧和二氧化碳潴留,改善临床症状,缩短住院治疗时间,值得临床广泛应用。Objective To observe the clinical therapeutic effect of bi - level positive airway pressure （BiPAP） on chronic obstructive pulmanary disease （COPD） with type Ⅱ respiratory failure, Methods 48 patients with COPD associated with type Ⅱ respiratory failure in this hospital were randomly divided into study group （ n = 24） and control group （ n = 24）. Corresponding fundamental therapy was given to patients in both groups, the patients in study group were given with noninvasive ventilation therapy by BiPAP machine, while patients in control group were given with bi - nasal catheter oxygen therapy, Reslts Patients in both groups experienced symptom relief in different levels, the therapeutic effect in patients of study group was faster and better, The mean duration of hospitalization was 22.5 ± 3. 2 days in control group while it was 11.6 ± 2.8 days in study group. Patients in bath groups were improved in findings of blood gas analysis, and the improvement in study group was obviously better than that of control group, and the difference was statistically significant （ P 〈 0.05 ）. Conclusion BiPAP noninvasive ventilation therapy in patients with COPD and type Ⅱ respiratory failure had following advantages： free of wound, exact therapeutic effect, rapid correction of hypoxia and carbon dioxide retention and improvement of clinical symptoms and significant reduction in duration of hospitalization. This kind of therapy is worthy to be recommended for clinical application.
