摘 要:目的:了解温州地区大肠埃希菌临床株中质粒介导的喹诺酮类耐药基因aac(6)′-Ib-cr的分布、耐药情况。方法:收集温州医学院附属第一医院环丙沙星耐药的大肠埃希菌临床株60株,应用PCR方法检测其aac(6)′-Ib基因,琼脂稀释法检测菌株的耐药情况,DNA测序aac(6)′-Ib-cr基因变异体,用接合传递试验方法验证细菌质粒介导的耐药性的可转移性。结果:60株大肠埃希菌临床株检出10株aac(6)′-Ib,其扩增产物进行测序,结果表明4株菌在第223位(T→C或T→A)发生变异,均携带aac(6)′-Ib-cr基因;这4株菌接合传递试验成功,临床株对喹诺酮类的耐药性传递给了受体株。结论:温州地区aac(6)′-Ib-cr介导的喹诺酮类耐药的大肠埃希菌较普遍存在。Objective:To investigate the distribution and instance of plasmid - midiated quinolone resistance of clinical isolates in Eschericha coli in Wenzhou. Methods:60 clinical isolates resistant to ciprofloxacin in Eschericha coli were collected from the First Affiliated Hospital of Wenzhou. Aac(6′) -Ib gene was detected by PCR, antimicrobial susceptibility test was examined by agar dilution test, DNA sequencing was used for indentification of aac (6′) - Ib - cr and conjugation experiment was proved to find ways of transmission of resistance. Results: The gene of aac ( 6′ ) - Ib was present in 10 isolate of 60. DNA sequencing for the 10 isolates found 4 of them contained nucleotide change at position 223 (T→C,T→A), meaning that they had aac (6′) - Ib - cr gene. Conjugation experiments were done in 4 isolates, and their resistance to quinolones was transimitted to the recipient isolates. Conclusion:Aac (6′) -Ib -cr gene of clinical isolates in Eschericha coli were popularly present in Wenzhou.
