摘　　要：目的:了解乙型肝炎病毒携带者血清中Pre-S1、HBsAg、ALT相关性与临床意义。方法:采用酶联免疫吸附方法对280例乙型肝炎病毒携带者进行HBV-M、Pre-S1检测,同时检测血清中ALT。结果:158例HBsAg(+)组,60例Pre-S1,阳性率38%,122例HBsAg(-)组,11例Pre-S1,阳性率9%,HBsAg(+)组与HBsAg(-)组,Pre-S1检测存在明显差异(P<0.01)。11例Pre-S1(+),6例ALT异常,阳性率55%,98例HBsAg(+),26例ALT异常,阳率性27%。Pre-S1(+)组与HBsAg(+)组,ALT异常存在明显差异(P<0.01),反映Pre-S1具有强的免疫原性,导致肝脏损害。结论:Pre-S1能够真实反映乙型肝炎病毒感染、复制的情况,特别是HBsAg(-)、HBeAg(-)条件下的乙型肝炎病毒情况,乙肝病毒损害肝细胞ALT升高,与HBsAg、Pre-S1引起的免疫反应相关。Objective：To investigate relationship and clinical sense among pre -S1 antigen, HBsAg, ALT. Methods：Using ELISA method to test the pre - S1 , HBsAg of 280 patients with hepatitis B and test ALT as well. Results ： 158 cases have HBsAg （ ＋ ）, 60 cases have pre -S1 antigen positive, and the positive rate was 38%. 122 cases have HBsAg （ - ） ,11 cases have pre - S1 antigen positive, and the positive rate was 9%. There was a great difference in pre -S1 antigen between the HBsAg（ ＋ ） Group and HBsAg（ - ） Group（P 〈 0.01 ）. there were 6 out of 11 cases pre -S1, in which its ALT was abnornal, with positive rate of 55%. there were 26 out of 98 cases HBsAg, in which its ALT was abnornal, with positive rate of 27%. There existed a great difference in abnornal rate between Group Pre - S1 （ ＋ ） and Group HBsAg（ ＋ ）, which indicated that Pre - S1 had strong immunity, thus causing liver damage. Conclusion：Pre - S1 can really reflect HBV infection and reproduction, especially the situation of virus under the condition of HBsAg（ - ） and HBeAg （ - ） , and that the rise of ALT, showing the fact that HBV virus has damaged liver cells, is closely related to the immune reaction caused by HbsAg and Pre - S1.
