摘 要:背景:目前不同影像学方法对椎动脉的检查仍然存在差异和争论,由于这些结果大多源于不同的病例或样本群,在很大程度上影响了对不同检查方法的客观评价。目的:以一组新鲜健康成人椎动脉标本为检测对象,对比分析数字减影血管造影、磁共振血管造影和计算机断层扫描血管造影在椎动脉检测中的准确性。设计、时间及地点:重复测量设计,于2003-08/2005-01完成。材料:11例健康成年人颈椎标本,来源于捐献尸体,年龄19-35岁,平均28.4岁,对比分析椎动脉20根。方法:应用高压注射器维持体外椎动脉标本的压力和速度在13.3kPa,4mL/s,使其尽可能接近正常活体椎动脉的生理充盈状态。依次将标本进行磁共振血管造影、计算机断层扫描血管造影、数字减影血管造影检测。主要观察指标:椎动脉直径。结果:磁共振血管造影、计算机断层扫描血管造影、数字减影血管造影得到的椎动脉直径分别为(0.311±0.058),(0.309±0.063),(0.316±0.059)cm,3种方法测量值差异无显著性意义(P〉0.05);3种方法测量左右两侧椎动脉直径差异也无显著性意义(P〉0.05)。结论:三种影像学方法在椎动脉临床直径测量方面没有明显区别。BACKBROUND: Different imaging detections for vertebral artery are still varied and controversial. Sample or case difference greatly influences the objective evaluation of various examination methods. OBJECTIVE: To analyze and compare the accuracy of digital subtraction angiography (DSA), magnetic resonance angiography (MRA) and CT angiography (CTA) in detecting vertebral artery from a group of healthy adults. DESIGN, TIME AND SETTING: Repetitive measurements were performed between August 2003 and January 2005. MATERIALS: Eleven healthy vertebral artery samples were from donated corpses with an average age of 28.4 years (range, 19-35 years), and 20 samples of vertebral artery served as control. METHODS: Using the high pressure syringe to control the pressure and velocity of in vitro vertebral artery to 13.3 kPa and 4 mL/s, which was similar to the condition of vertebral artery in vivo. The samples of vertebral artery were detected by DSA, MRA and CTA. MAIN OUTCOME MEASURES: Diameter of vertebral artery. RESULTS: The diameter of vertebral artery was (0.31 1±0.058), (0.309±0.063), and (0.316±0.059) mm by DSA, MRA and CTA. There were no significant differences among the three methods (P 〉 0.05). No significant differences were found in diameter of left and right vertebral artery by three methods (P 〉 0.05). CONCLUSION: There are no significant differences in the diameter of vertebral artery by DSA, MRA and CTA.
