摘  要:学术背景:近些年组织工程和基因技术开辟了修复软骨关节的新途径,但目前尚在研究阶段,有较多问题需解决。骨形态发生蛋白可促进软骨生成和修复,在关节软骨修复上有着很好的前景。目的:总结分析骨形态发生蛋白在关节软骨修复过程中的作用及可能的应用前景。资料来源:应用计算机检索北京大学医学部"ScienceDirect Onsite"数据库和"Medline"数据库1996-01/2007-05有关英文文献,检索词"BMPs,cartilage repair/chondrocyte",限定文章语言种类为英文;同时应用中国期刊全文专题数据库和中国科技期刊数据库1996-01/2007-05有关中文文献,检索词"骨形态发生蛋白,关节软骨修复",限定文章语言种类为中文。对资料初审,纳入标准:①与目前关节软骨修复的现状和问题有关的文献。②有关骨形态发生蛋白对关节软骨修复的作用研究的文章。文献评价:收集到符合上述要求的文献61篇,除去无关研究和重复性研究,采用40篇。资料综合:临床上修复关节软骨有几种主要方式,如软骨或骨细胞移植、骨膜移植、自体软骨细胞移植等,自体软骨细胞移植是目前反映最好的一种方式,但仍存在问题。骨形态发生蛋白对关节软骨修复具有促进作用,有促进软骨分化、基质形成,并抑制软骨细胞去分化等作用。结论:骨形态发生蛋白在促进软骨修复方面的作用有良好应用前景,随着研究的不断深入,关节软骨修复这个临床上的难题将有望被突破。BACKGROUND: A new method to repair cartilage joint is developed by present tissue engineering and gene technique. It is still in the research stage and a lot of problems should be solved. Bone morphogenetic proteins (BMPs) can enhance the cartilage repair, and may be important in the future for cartilage defect. OBJECTIVE: To summarize and analyze the effect and tendency of the application of BMPs in cartilage repair. RETRIEVAL STRATEGY: Online search of ScienceDirect Onsite and Medline database was performed for related English articles published from January 1996 to May 2007 with the key words of "BMPs, cartilage repair/chondrocyte". Simultaneously, China Journal Full-text Database (CJFD) and China Scientific Journal Database were also searched for relevant articles published between January 1996 and May 2007 with the same key words in Chinese. The data were primarily screened. Inclusive criteria: ① articles of the constant situation of articular tissue repair in clinical practice and problems, ②researches on effect of BMPs on articular tissue repair. LITERATURE EVALUATION: Totally 61 articles meeting the inclusive criteria were selected, and 40 articles remained after excluding irrelevant ones and repetitive ones. DATA SYNTHESIS: There are several ways clinically on articular cartilage repair, such as periosteal transplantation, osteochondral transplantation, autologous chondrocyte transplantion (ACT). ACT is the most prevalent practice at present, though some problems still exist. BMPs have the effect of promoting chondrogenous repair, chondrogenous differentiation, matrix forming, and inhibiting the dedifferentiation process. CONCLUSION: With further study on the effects of BMPs on chondrogenesis and other process that benefits the chondrocyte repair, BMPs will open a new and prospective era of chondrocyte repair.
