摘 要:目的:探讨转化生长因子β1(TGF-β1)在病理性瘢痕形成过程中的作用及其对病理性瘢痕成纤维细胞增殖和凋亡的影响。方法:收集病理性瘢痕标本58例、正常瘢痕标本42例和正常皮肤标本30例。采用免疫组织化学方法检测成纤维细胞TGF-β1、增殖细胞核抗原(PCNA)的表达,以及TUNEL法检测成纤维细胞的凋亡水平。结果:病理性瘢痕组织中TGF-β1表达水平高于正常瘢痕组织(P<0.05),正常皮肤组织中TGF-β1表达水平明显低于病理性瘢痕及正常瘢痕组织(P<0.05)。而PCNA在病理性瘢痕成纤维细胞中表达水平与正常瘢痕及正常皮肤相比明显增高(P<0.05),正常瘢痕中成纤维细胞PCNA的表达亦高于正常皮肤(P<0.05)。另一方面,病理性瘢痕中成纤维细胞凋亡水平较正常瘢痕降低(P<0.05),而正常皮肤与正常瘢痕间差异无显著性(P>0.05)。比较病理性瘢痕组织中成纤维细胞增殖指数、凋亡指数与TGF-β1间的关系发现,细胞增殖指数与TGF-β1表达水平呈正相关关系(P<0.05),凋亡指数则与TGF-β1呈负相关关系(P<0.05)。结论:TGF-β1可能是通过调节瘢痕组织中成纤维细胞的增殖和(或)凋亡水平,来实现对瘢痕形成的调节。TGF-β1表达水平的失控可能是病理性瘢痕形成的重要诱因之一。Objective To investigate the role of TGF-β1 in the occurrence and development of abnormal scar and their modulation to proliferation and apoptosis of fibroblast in abnormal scar. Methods Tissue specimens were obtained from 58 abnormal scars, 42 normal scars and 30 normal skins. TGF-β1 and PCNA were stained by immunohistochemistry and terminal deoxynucleotide transferase mediated dUTP nick-end labeling (TUNEL) was used to detect fibroblast apoptosis, Results Immunohistochemical staining results suggested that the expression of TGF-β1 was significantly higher in abnormal scar than that of normal scar (P 〈 0.05). The level of TGF-β1 expression was significantly lower in normal skin than that of abnormal scar and normal scar (P 〈 0.05). Compared with normal scar and normal skin the expression of PCNA in fibroblast of abnormal scar increased significanfly(P 〈 0.05). Compared with normal skin the expression of PCNA in fibroblast of normal scar increased significantly (P 〈 0.05). Compared with normal scar the level of fibroblast apoptosis in abnormal scar decreased significantly (P 〈 0.05), but there was no significant difference between normal scar and normal skin (P 〉 0.05). There was positive correlation between the proliferation index and the expression of TGF-[31 in fibroblast of abnormal scar (P 〈 0.05). There was negative correlation between the apoptosis index and the expression of TGF-β1 in fibroblast of abnormal scar (P 〈 0.05). Conclusions TGF-β1 is associated closely with the occurrence of abnormal scar. TGF-β1 may regulate the occurrence and development of scar by means of regulating the proliferation and/or the level of apoptosis of fibroblast in scar tissue.
