摘　　要：目的探讨老年人左束支传导阻滞时QRS波时限变化对心脏功能不全的影响。方法对2003年至2006年住院确诊的左束支传导阻滞合并心脏功能不全的老年患者160例,根据心电图QRS波时限分为A、B、C组,分别测定各组入院时及治疗后心电图QRS波时限,心脏彩色超声心动图,对比分析各组治疗前、后QRS波时限变化对左心室功能参数的影响。结果B、C组缺血性心脏病?陈旧性前壁心肌梗死?扩张型心肌病患者均明显高于A组(P<0.05);重度心衰在B、C组明显高于A组,C组二尖瓣反流明显多于B、A组,P<0.05,差异具有统计学意义。结论老年人左束支传导阻滞时QRS波时限越长,心肌电活动紊乱越重,预后越差。Objective To explore influential of the QRS complex duration on cardiac insufficiency in old patients with left bundie branch block. （LBBB）. Methods 160 cases of cardiac insufficiency in old patients with LBBB,which admitted hospital in 2003 - 2006 ,were analyzed, to be divided into groups A,groups B,groups C. Comparing the change of the QRS complex duration and data of left ventricular from U CG. Results Comparing with CLBBB （group B, group C） the incidence rate of ischemic heart disease, obsolete anterior myocarsial infarction, dilated cardiomyopathy and grave congestive heart failure, mitral incompetence than group A （all P 〈 0. 05）. Before treatment, the LVD, LVEDV, LVESV were improved obviously and the LVEF, FS were decreased signigicantly. Comparing with the same group , after treatment the QRS complex duration was not more changed ;but group B. group C the LVEDV , LVESV were reduced and the LVEF, FS were improved than before treatment. Conclusion The QRS complex duration is longer and longer,cardiac insufficiency is seriously.
