血清ANCA检测在原发性小血管炎中的临床意义

摘　　要：目的检测原发性小血管炎患者血清抗中性粒细胞胞浆抗体(ANCA)并探讨其临床意义。方法应用间接免疫荧光(IIF)法检测50例原发性小血管炎疾病患者血清中ANCA的检出率。结果50例原发性小血管炎血清中,韦格纳肉芽肿(WG)阳性率95%,为C-ANCA阳性;显微镜下多血管炎(MPA)阳性率50%,为P-ANCA阳性;过敏性紫癜(AP)阳性率37.5%,为P-ANCA阳性;白塞病(BD)3例,ANCA均为阴性;未分型原发性小血管炎(UCPSV)阳性率66.67%,其中C-ANCA 4例,P-ANCA 6例。结论抗中性粒细胞胞浆抗体在原发性小血管炎中,部分疾病与抗中性粒细胞胞浆抗体密切相关,尤其是韦格纳肉芽肿阳性率95%,提示C-ANCA可作为韦格纳肉芽肿血清学标志性抗体,ANCA检测对血管炎的发病及诊断具有一定的作用。Objective To disscuss the clinical significance of antineutrophil cytoplasmic antibody （ANCA） in primary small vasculitis. Methods The serum ANCA was tested by indirect immuno - fluorescence（IIF） in 50 patients. Results The positive rate of serum ANCA in wegner＇ s granulomatosis（WG） was 95% ,it was C - ANCA positive;50% in microscopicpoly angiitis（MPA） ,which was P -ANCA positive;37. 5% in anaphylactoid purpura（AP） ,which was P- ANCA positive;66. 7% in un -classed primary systemic vasculitis （ UCPSV ）, of which 4 was C - ANCA positive, 6 was P - ANCA positive ; the ANCA in 3 behcet＇ s disease （BD） were negative. Conclusion The detection of serum ANCA showed the important diagnostic significance in ANCA associated small vasculitis, especially in WG, the C - ANCA can be regarded as a serum marker of diagnose it.
