应用国产封堵器治疗室间隔缺损的疗效及安全性观察

摘　　要：目的:观察国产室间隔缺损封堵器治疗室间隔缺损的安全性及近期疗效。方法:2005年3月至2007年8月采用国产封堵器对32例(男18例,女14例)室间隔缺损患者进行封堵治疗。患者年龄2～45岁,平均(10.5±5.9)岁,经胸超声提示室间隔缺损的直径为(2.0～12)mm,平均(5.9±2.5)mm,心室造影示室间隔缺损的直径为(1.8～13)mm,平均(5.4±3.1)mm。在透视及超声监测下通过建立股动静脉轨道、经右心系统释放封堵器。分别于术后3个月进行随访。结果:32例患者封堵器置入均成功,成功率100%。术后超声及造影示微量残余分流4例,1个月内消失。术后第二天出现完全性左束支传导阻滞2例,1周内消失。结论:应用国产室间隔缺损封堵器治疗室间隔缺损是安全有效的,近期效果良好,但中、远期疗效尚需更大规模的临床观察。Objective：To evaluate the feasibility and efficacy of transcatheter closure of the new home-made ventricular septat defects occluder on ventricular septal defects.Methods：From March 2005 to August 2007 ,thirty two patients（18 males, 14 females）with VSD underwent an attempt of catheter closure using occluder.The mean age of patients was （10.5±5.9） years （ranged from 2 to 45 years）;The mean diameter of VSD measured by transthoracic echocardiography （TIE） was （5.9±2.5）mm （ranged from 2.0 to 12 mm）.All patients underwent left ventriculography before catheter closure.The mean diameter of VSD was （5.4±3.1）mm （ranged from 1.8 to 13 mm）,and underwent closure under TTE guidance. All patients were followed up 3 months after the procedures. Results：The device was deployed successfully in all patients.There was trace residual shunt in 4 patients,and all disappeared within 1 month. Complete left bundle branch block （LBBB） was found on electrocardiogram 1 week post-operation in 2 patients,which disappeared within 1 week.Conclusions：Transcatheter closure of ventricular septal defects using this new occluder is relatively safe and effective, early results are good.But patients should be chosen very strictly.Further clinical trial are underway to assess the long term results.
