摘  要:背景:特重烧伤患者自体皮源匮乏,只有开发新的皮源才能缓解这一难题。目的:在微粒皮移植的基础上稍作改进,拟观察薄中厚超微粒头皮移植修复特重烧伤患者的临床效果。设计、时间及地点:自体对照观察,于2005-01/2008-01在唐山钢铁公司医院烧伤科、唐山工人医院烧伤科、山西省烧伤救治中心、唐山路北红十字医院、唐山二五五医院烧伤科及唐山市第三医院烧伤科完成。对象:选取特重烧伤患者102例,平均烧伤面积为体表总面积的(60.55±3.21)%,Ⅲ度烧伤面积为体表总面积的(51.74±2.98)%,手术面积为体表总面积的(52.19±5.44)%。方法:每例患者选取较为对称、创面性质相同且采用不同厚度头皮微粒移植的2处Ⅲ度烧伤切痂创面进行对照,分别记为薄中厚超微粒头皮移植组及常规微粒头皮移植组。薄中厚超微粒头皮移植组移植薄中厚超微粒头皮,供受皮区面积比为1∶20;常规微粒头皮移植组移植正常头皮微粒,供受皮区面积比为1︰10。主要观察指标:应用网格计数法统计头皮移植后3,4,5周末的创面愈合率。结果:移植后第3,4,5周末两组创面愈合率基本相近(P均>0.05)。结论:薄中厚超微粒头皮移植是一种有效的治疗大面积特重烧伤的方法,与传统常规头皮微粒移植相比,在供皮面积相同、受皮区面积增加1倍的情况下创面愈合率无明显差异。BACKGROUND: Being lack of the source of autologous skin, only to develop new source of skin can solve this problem. OBJECTIVE: To explore the clinical effect of the ultramicronized thin middle thick head skin transplantation for treating extensively deep burn after modifying micronized skin transplantation. DESIGN, TIME AND SETTING: The autologous control observation was performed at the Department of Bums, Hospital of Tangshan Steel and Iron Co.Ltd, together with Department of Bums, Hospital of Workers; Bums Treatment Center of Shanxi Province; Red Cross Hospital of Lubei Area; Department of Bums, Tangshan 255 Hospital, and Department of Burns, Tangshan Third Hospital from January 2005 to January 2008. PARTICIPANTS: In 102 serious burn patients, average burn area for total body surface (60.55 ±3.21) %,Ⅲ burn area for total body surface (51.74 ± 2.98) %, surgery area for total body surface (52.19 ± 5.44) %. METHODS: In each patient, two symmetrical spots of Ⅲ burn area were selected and were divided equally into two groups. The experimental group transplanted the ultrarnicronized thin middle level thick skin of head, the proportion of accepted area to providing was 1:20; while the control group transplanted the normal micro-skin of head, its proportion was h 10. MAIN OUTCOME MEASURES: Wound healing rates were analyzed at 3, 4 and 5 weeks after head skin transplantation by grid counting. RESULTS: After transplantation, the recovery rate of injured area was similar at 3, 4 and 5 weeks (P 〉 0.05). CONCLUSION: The ultramicronized thin middle thick head skin transplantation is one effective treatment method to extensively deep burn patient's, and there is no obvious difference compared with traditional normal micro-skin transplantation as the proportion of accepted area to providing is doubled.
