不稳定性心绞痛患者血管内皮功能及普罗布考干预研究

摘　　要：目的:观察普罗布考对不稳定性心绞痛患者内皮依赖舒张功能、血氧化低密度脂蛋白(ox-LDL)和高敏C反应蛋白(hs-CRP)水平的影响。方法:将53例不稳定性心绞痛患者随机分为普罗布考组和常规治疗组,治疗前、治疗3个月后应用高频超声测肱动脉内皮依赖性舒张功能(FMD),并观察外周血ox-LDL及hs-CRP等指标变化。结果:治疗后普罗布考组FMD较治疗前及常规治疗组治疗后均明显升高(P<0.01),血浆ox-LDL及hs-CRP水平较治疗前及常规治疗组治疗后均明显降低(P<0.01或P<0.05)。普罗布考组治疗后血浆ox-LDL与FMD呈负相关(r=-0.651,P<0.01)。结论:普罗布考具有较强的抗氧化及抗炎症作用并可改善不稳定心绞痛患者内皮依赖舒张功能。Objective： To evaluate the effect of probucol on the vascular endothelial function and the oxide low-density lipoprotein （ox-LDL） lever in patients with unstable angina. Methods： Fifty-three unstable angina patients were divided randomly into two groups, probucol treatment group and routine treatment group. The brachial arterial hyperemia-induced flow-mediated dilation （FMD） and sublingual nitroglycerin-mediated vasodilation （NTG） were measured by ultrasound cardiograph before and after 3 months of probucol treatment. The plasma levels of ox-LDL and high-sensitivity C-reactive protein （hs-CRP） were examined as well. Results： Compared with pretreatment and routine treatment group,the level of FMD was significantly increased （P 〈 0.01 ） ,while the level of blood ox-LDL （P 〈 0.01 ） and hs-CRP （P 〈 0.05） were marked decreased after 3 months treatment in probucol treatment group, compared with pretreatment and routine treatment group. The liner correlation analysis showed that there was a negative correlation（r = -0.651, P 〈 0.01 ）between plasma ox- LDL and FMD in patients with unstable angina after probucol treatment. Conclusion：Probucol could reduce the plasma level of ox-LDL and hs-CRP significantly, suggesting its effects of anti-oxidation and anti-inflammation. The endothelial function could be improved after probucol therapy in patients with unstable angina.
