乳腺纤维腺瘤与囊肿的X线特征分析

摘　　要：目的总结乳腺纤维腺瘤与囊肿的X线特征,提高诊断及鉴别能力。方法回顾28例纤维腺瘤和26例囊肿的X线特征及BI-RADS分级。结果两者均多为类圆形(45.45%和41.18%)或圆形(12.12%和23.53%),不典型者为斑片状密影(21.21%和26.47%)。病变边界多为部分清晰(45.45%和55.88%)或完全清晰(33.33%和14.71%),少量不清晰(21.21%和29.41%)。大部分归为BI-RADS 2级(39.28%和23.08%)和3级(23.08%和53.85%)。两者各项X线特征比较无明显差异(P值>0.05)。结论纤维腺瘤和囊肿多具X线特征,易作出BI-RADS2、3级的诊断,但需注意其不典型表现。单凭X线征象鉴别两者是困难的。Objective To investigate the mammographic features of breast fibroadenoma and cyst so that to improve the diagnostic level. Methods Mammographic features and BI- RADS range of 33 fibroadenomas （26 cases）and 34 cysts（28 cases） were reviewed retrospec- tively. Results Most of breast fibroadenomas and cysts were oval（ 45.45% and 41.18% ） or round（ 12.12% and 23.53% ） , a few of them were lobular（ 21.21% and 8.82% ） , the atypical exhibition of the masses were patchy（21.21% and 26.47% ）. Most of the lesions had definite border in part（45.45 % and 55.88% ） or completely（33.33% and 14.71% ） , but a few of them had hazy border（21.21% and 29.41% ）. Most of both of the lesions were in grade 2（ 39.28% and 23.08% ）or grade 3（ 23.08% and 53.85% ） ,a few of them were ranged in grade 4（ 14.29% and 23.08% ）. No obvious difference was seen in X -ray features between the breast fibroadenoma and cyst （ P 〉 0.05 ）. Conclusion Breast fibroadenoma and cyst have certain mammographic features, most of them can be confirmed easily in BI - RADS grade 2 or grade 3. But the atypical exhibition should be regarded. It is difficult to make a correct differentiation between breast fibroadenoma and cyst by only mammograms.
